    Hubungan antara Feritin Serum dengan Inflamasi, Nutrisi dan Besi pada Pasien-Pasien Hemodialisis Reguler di Medan, Sumatera Utara

    Abstract “Association Between Serum Ferritin and Measures of Inflammation, Nutrition and Iron in Haemodialysis Patients in Medan, North Sumatera” Melati Silvanni Nasution, Harun Rasyid Lubis, Salli Roseffi Nasution Division of Nephrology and Hypertension Department of Internal Medicine Medical Faculty, University of Sumatera Utara H. Adam Malik General Hospital Medan Background : Serum ferritin is a frequently used marker of iron status in hemodialysis patients. Iron administration is to be withheld for ferritin values > 800 ng/ml according to K/DOQI guidelines and > 500 ng/ml according to PERNEFRI guidelines. We hypothesized that such non iron related factros as elements of the malnutrition-inflammation complex syndrome (MICS) may increase serum ferritin concentration indepedently of iron status. Methods : This was an observational cross sectional study with 83 maintenance haemodialysis patients that received ESA. We examined the antropometry, laboratory (iron status, inflammation status, albumin) and recorded the MIS score. Results : The 83 subject comprised 48 men (57.8%) and 35 women (42.2%) with mean (± SD) of age 52.57 ± 11.52 years old. Mean of serum iron was 114.87±97.9 ng/ml and mean of serum ferritin was 1253.268 ng/ml. This study showed significant correlation between serum ferritin and both markers of inflammation and iron status independent of each other. After dividing the 83 patients into two groups of serum ferritin cut off of 500 ng/ml, we found almost all the patients had serum ferritin > 500 ng/ml Conclusions : There were positive associations between serum ferritin and some markers of inflammation and malnutrition in a group of maintenance haemodialysis patients in Adam Malik and Dr Pirngadi hospital in Medan, North Sumatera.     Abstrak ”Hubungan Antara Feritin Serum dengan Inflamasi, Nutrisi dan Besi pada Pasien-pasien Hemodialisis Reguler di Medan, Sumatera Utara ” Melati Silvanni Nasution, Harun Rasyid Lubis, Salli Roseffi Nasution Divisi Nefrologi dan Hipertensi Departemen Ilmu Penyakit Dalam Fakultas Kedokteran Universitas Sumatera Utara RSUP.H.Adam Malik Medan Pendahuluan : Serum feritin sering digunakan sebagai penanda status besi pada pasien-pasien hemodialisis. Pemberian besi biasanya ditunda jika kadar feritin serum > 800 ng/ml berdasarkan guideline K/DOQI dan > 500 ng/ml berdasarkan guideline PERNEFRI. Kami berhipotesa bahwa faktor-faktor yang tidak berhubungan dengan besi seperti bagian-bagian dari malnutrition-inflammation complex syndrome (MICS) dapat meningkatkan feritin serum selain karena status besinya Bahan dan Cara : Penelitian ini merupakan penelitian observasional dengan metode pengukuran cross sectional, melibatkan 83 pasien GGK yang menjalani HD reguler yang telah mendapatkan ESA. Kepada subjek dilakukan pengukuran antropometri, pemeriksaan status besi, inflamasi, albumin serta pencatatan skor MIS.. Hasil : Dari 83 subjek terdiri dari 48 pria (57.8%) dan 35 wanita (42.2%) dengan rerata (± SD) usia 52.57 ± 11.52 tahun. Rerata kadar besi sebesar 114.87±97.9 ng/ml dengan nilai rata-rata feritin serum 1253.268 ng/ml pada seluruh 83 pasien. Terdapat korelasi yang signifikan antara feritin serum dengan penanda inflamasi dan status besi yang tidak berhubungan satu sama lain. Feritin berkorelasi positif dengan CRP serum, transferin dan serum iron. Skor MIS berkorelasi positif dengan CRP serum sedangkan albumin berkorelasi negatif dengan CRP serum. Setelah membagi 83 pasien menjadi 2 kelompok berdasarkan cut off feritin serum 500 ng/ml, feritin serum > 500 ng/ml lebih banyak dijumpai Kesimpulan : Terdapat hubungan yang positif antara feritin serum dengan beberapa penanda inflamasi dan malnutrisi pada kelompok pasien-pasien hemodialisis reguler di RSUP Haji Adam Malik dan RSUD Dr Pirngadi Medan. Kata Kunci : GGK, feritin, inflamasi, besi, malnutrition-inflammation complex syndrome, protein-energy malnutrition.
