dc.description.abstract | Background: Hyperbilirubinemia is one of the most common clinical
phenomenon found in newborns. Phototherapy is standart treatment for
lowering bilirubin levels in neonates. Intensive phototherapy produces a more
rapid decline in the bilirubin serum levels than standard phototherapy, it is
possible a greater rebound might occur.
Objectives: To determine the outcome of post-phototherapy rebound after
single and double phototherapy are discontinued, and to establish the
comparison between post-phototherapy bilirubin rebound after single
phototherapy and double phototherapy.
Methods: An open randomized controlled trial was conducted at H. Adam
Malik hospital and Dr. Pirngadi hospital Medan in August 2009 until January
2010 and was performed on 81 neonates with indirect hyperbilirubinemia.
Measurement and observation of plasma total bilirubin level were conducted
within 12 hours and after 24 hours after phototherapy is discontinued.
Rebound bilirubin serum level is the increment of bilirubin serum level of
about 1 – 2 mg/dL after the phototherapy is discontinued
Results: Based on 24 hour observation after single phototherapy is
discontinued, it is found that 1 neonate (2.7%) has billirubin serum level
increase of about 1 – 2 mg/dL after phototherapy is discontinued. On the
other hand, observation for double phototherapy group shows that 4
neonates (10.8%) have billirubin serum level increase of about 1 – 2 mg/dL
after phototherapy is discontinued. Fisher Exact Test did not reveal any
significant difference to rebound bilirubin serum levels in both groups (P =
0.358)
Conclusions: Rebound bilirubin serum levels after single and double
phototherapy may occur in some cases associated with increased bilirubin
production that keep on going. There were no significant differences of
rebound bilirubin serum levels found after single or double phototherapy. | en_US |
dc.description.abstract | Latar belakang: Hiperbilirubinemia merupakan salah satu fenomena klinis
yang paling sering ditemukan pada bayi baru lahir. Fototerapi merupakan
terapi standar yang digunakan untuk menurunkan kadar bilirubin pada
neonatus. Fototerapi intensif dapat menurunkan kadar bilirubin serum lebih
cepat dibandingkan fototerapi standar, kemungkinan yang lebih besar
terhadap peningkatan kembali kadar bilirubin serum (rebound) dapat saja
terjadi.
Tujuan: Untuk mengetahui kejadian peningkatan kembali kadar bilirubin
serum setelah fototerapi tunggal dan fototerapi ganda dihentikan dan untuk
membandingkan nilai peningkatan kembali kadar bilirubin serum setelah
fototerapi tunggal dan fototerapi ganda
Metode: Uji klinis terbuka, dilakukan di RS.H.Adam Malik dan RS. Dr.
Pirngadi Medan. Penelitian dimulai bulan Agustus 2009 sampai Januari 2010
terhadap 81 neonatus cukup bulan sesuai masa kehamilan yang menderita
hyperbilirubinemia indirek. Dilakukan pemeriksaan dan pemantauan kadar
bilirubin total awal, 12 jam dan setelah 24 jam fototerapi dihentikan.
Peningkatan kembali kadar bilirubin serum atau rebound adalah peningkatan
kadar bilirubin serum 1 – 2 mg/dL setelah fototerapi dihentikan.
Hasil: Dari hasil pemantauan 24 jam setelah fototerapi tunggal dihentikan,
Dijumpai 1 neonatus (2.7%) yang memiliki peningkatan kadar bilirubin serum
1 – 2 mg/dL setelah fototerapi dihentikan. Dan pada kelompok fototerapi
ganda dijumpai 4 neonatus (10.8%) yang memiliki peningkatan kadar bilirubin
serum 1 – 2 mg/dL setelah fototerapi. Dengan menggunakan uji fisher exact
tidak ditemukan adanya perbedaan yang signifikan terhadap peningkatan
kembali kadar bilirubin serum pada kedua kelompok
Kesimpulan: Peningkatan kembali kadar bilirubin serum setelah fototerapi
tunggal dan ganda dapat saja terjadi pada beberapa keadaan yang
berhubungan dengan peningkatan produksi bilirubin yang terus berlangsung.
Tidak dijumpai perbedaan yang signifikan terhadap peningkatan kembali
kadar bilirubin serum setelah fototerapi tunggal maupun fototerapi ganda | en_US |
