dc.description.abstract | Background and Objective: Pain after surgery is one of the important problems that accompany the process of operation, so that a strong analgesia is needed in post-surgical period. NSAID medicines are popular as analgesia because this drug can reduce opioid analgesic requirements and levels of inflammatory mediators in the area of trauma but did not cause sedation. Opioids as analgesics after surgery laparotomy has also been widely used. Method of delivery through skin absorption is one way that has been widely used. The combination of NSAIDs and opioids is a balance of analgesia. This study aimed to compare the combination of ketorolac 30 mg and fentanyl patch 12.5 ug / h, ketorolac 30 mg and fentanyl patch 25 ug / h, ketorolac 30 mg and placebo patch for pain management after laparotomy surgery.
Methods: After obtaining approval from the ethics committee of the Faculty of Medicine, USU, 30 samples were collected, men and women, from age 16-50 years with ASA physical status 1-2, VAS preoperatively 0-1, undergoing elective laparotomy surgery under general anesthesia with intubation orotrakea at the RSUP H. Adam Malik Hospital in Medan networking. The samples then were divided randomly into three groups with each 10 subjects. This research used double-blind randomized controlled clinical trial. Group A received ketorolac 30 mg and fentanyl patch 12.5 g / hour, and group B received ketorolac 30 mg and fentanyl patch 12.5 ug / hr, and Group C receive ketorolac 30 mg and placebo patch. Systolic blood pressure, diastolic blood pressure, mean arterial pressure, breath frequency and pulse frequency, VAS values were recorded and analyzed data with ANOVA and chi square.
Results: VAS values after the administration of analgesic there were significant differences between group A, group B with group C. The results respectively were found with ANOVA p = 0:16, p = 085., P = 0.001, p = 0.001, p = 0.0001, p = 0.00, p = 0.001, p = 0.001.
Conclusion: The results of the study there were significant differences between the two groups with fentanyl patch statistically placebo. Patients given fentanyl patches get smaller VAS values | en_US |
dc.description.abstract | Latar belakang dan Objektif : Nyeri paska bedah merupakan salah satu masalah penting yang menyertai proses operasi, sehingga analgesia yang kuat sangat dibutuhkan pada periode paska bedah. Obat-obat AINS sudah popular sebagai analgesia kerena obat ini dapat menurunkan kebutuhan analgetik opioid dan kadar mediator inflamatori pada daerah trauma tetapi tidak menyebabkan sedasi. Opioid sebagai analgetik setelah operasi laparatomi juga telah banyak dipakai. Metode pemberian melalui penyerapan kulit merupakan salah satu cara yang telah banyak digunakan. Kombinasi antara AINS dan opioid merupakan balans analgesia. Penelitian ini ditujukan untuk membandingkan kombinasi ketorolak 30 mg dan fentanyl patch 12,5 μg/jam, ketorolak 30 mg dan fentanyl patch 25 μg/jam , ketorolak 30 mg dan placebo patch untuk penatalaksanan nyeri setelah pembedahan laparatomi.
Metode : Setelah mendapat persetujuan dari komite etik Fakultas Kedokteran USU, 30 sampel dikumpulkan, pria dan wanita, dari umur 16-50 tahun dengan status fisik ASA 1-2, VAS preoperasi 0-1, yang menjalani pembedahan elektif laparatomi dengan anestesi umum dengan intubasi pada Rumah Sakit Umum Pusat H. Adam Malik dan Rumah sakit jejaring di kota Medan. Sampel kemudian dibagi secara acak menjadi tiga kelompok dengan masing-masing 10 subjek. Penelitian ini menggunakan uji klinis acak terkontrol tersamar ganda. Kelompok A mendapat ketorolak 30 mg dan fentanyl patch 12,5 μg/jam, dan kelompok B mendapat ketorolak 30 mg dan fentanyl patch 12,5 μg/jam, serta Kelompok C mendapatkan ketorolak 30 mg dan placebo patch. Tekanan darah sistolik, tekanan darah diastolik, tekanan arteri rerata, frekwensi nafas dan frekwensi nadi, nilai VAS dicatat dan diukur.Semua data dianalisa menggunakan uji ANOVA dan chi square. Hasil : Didapatkan nilai VAS sesudah pemberian analgetik ada perbedaan bermakna antara kelompok A, kelompok B dengan kelompok C. Hasil penelitian secara berturut-turut dengan uji ANOVA didapati p=0.16 , p=085., p=0.001, p=0.001, p=0.0001, p=0.00, p=0,001, p=0,001.
Kesimpulan : Dari hasil penelitian ada perbedaan bermakna diantara kedua kelompok fentanyl patch dengan placebo secara statistik. Pasien yang diberikan fentanyl patch mendapatkan nilai VAS yang lebih kecil | en_US |
