    Efek Teknik Penempatan Resin Komposit dan Penggunaan Sistem Matriks terhadap Kebocoran Mikro pada Restorasi Kelas II Resin Komposit Berbentuk Saucer

    Abstract
    One of the problems of Class II composite resin restorations is microleakage at the gingival margin of proximal. The saucer-shaped Class II cavity design can reduce the disadvantages of the box-shaped Class II cavity design, such as extensive loss of sound tissue, large occlusal reduction, and inadequate gingival enamel margins. Composite resin filling technique also influence in reducing polymerization shrinkage which will cause microleakage. Besides, the use of matrix also contribute to achieve good marginal ridge. This study aims to examine the effect of composite resin filling techniques and the use of matrix systems on microleakage of saucer-shaped Class II restorations. Mandibular premolar specimens were prepared in the respect of saucer shaped Class II cavity design, then were divided in to eight groups according to composite resin filling techniques (bulk or incremental), the use of matrix systems (circumferential or sectional), and whether load is provided or not. Specimens were immersed in saline for 24 hours, then were thermocycled, later were immersed in 2% methylene blue for 24 hours. Micro gap were measured by scoring dye penetration in specimens that were sectioned mesio-distally under a stereomicroscope with 20x magnification. Furthermore, visual analysis was performed under Scanning Electron Microscope to examine the microleakage in detail. Statistical analysis with Kruskal Wallis test and Mann-Whitney test demonstrated that bulk and incremental filling techniques were not significantly different (p>0.05) on microleakage. This study also demonstrated that the use of matrix was significantly different (p<0.05) on microleakage in different filling techniques, whether load was provided or not. This study showed that the use of matrix was important in saucer-shaped Class II restorations.
     
    Salah satu masalah restorasi Kelas II resin komposit adalah kebocoran mikro margin gingiva proksimal. Desain kavitas Kelas II berbentuk saucer dapat mengurangi kerugian desain kavitas Kelas II berbentuk box, seperti kehilangan jaringan sehat yang ekstensif, pengambilan oklusal yang luas, dan tepi email gingival yang tidak adekuat. Teknik penempatan resin komposit juga berpengaruh mengurangi pengerutan akibat polimerisasi yang menyebabkan kebocoran mikro. Selain itu, penggunaan matriks juga berperan dalam menghasilkan tepi marginal yang baik sehingga mengurangi kebocoran mikro. Penelitian ini bertujuan untuk melihat pengaruh teknik penempatan resin komposit dan penggunaan sistem matriks terhadap kebocoran mikro pada restorasi Kelas II berbentuk saucer. Sampel gigi premolar mandibula dipreparasi sesuai dengan desain kavitas Kelas II saucer, kemudian dibagi dalam delapan kelompok perlakuan berdasarkan teknik penempatan resin komposit (bulk atau incremental), penggunaan sistem matriks (sirkumferensial atau seksional), dan ada tidaknya pemberian gaya (load). Sampel direndam dalam saline selama 24 jam, kemudian dilakukan thermocycling, selanjutnya direndam dalam larutan methylene blue 2% selama 24 jam. Pengukuran celah mikro dengan melihat penetrasi zat warna pada sampel yang dibelah secara mesio-distal melalui stereomikroskop perbesaran 20x. Selanjutnya, dilakukan pengamatan dengan Scanning Electron Microscope untuk melihat kebocoran mikro secara mendetail. Analisis statistik dengan Kruskal-Wallis Test dan Mann-Whitney Test menunjukkan bahwa teknik penempatan bulk dan incremental tidak berbeda secara bermakna (p>0.05) terhadap kebocoran mikro. Penelitian juga menunjukkan bahwa penggunaan matriks berbeda secara bermakna (p<0.05) terhadap kebocoran mikro pada teknik penempatan yang berbeda, baik diberi gaya (load) maupun tidak. Penelitian ini menunjukkan bahwa penggunaan matriks penting pada restorasi kelas II berbentuk saucer.
