Background. The effect of iron on weight and height increasing in children with iron deficiency anemia (IDA) has been investigated can improve growth. Possibility of coexistence of malnutrition and iron deficiency frequently found. Objective. To investigate the effect of iron on nutritional status in children with IDA treated with iron. Methods. A randomized placebo-controlled clinical trial study was conducted at Bilah Hulu, a subdistrict of Labuhan Batu on November 2006 – February 2007. Iron deficiency anemia was diagnosed if there were anemia, MCHC 220 and Mentzer index >13. Elementary school children (8 to12 years old) with IDA were randomly assigned to a daily therapy of 6 mg iron/kg/day or placebo groups for three months. The nutritional status were evaluated with anthropometric assesment before and after intervention. Results. There were 111 children (37,2%) among 300 children who recruited suffered from IDA. After intervention, 108 children completed the therapy, the iron and placebo groups had difference on mean of hemoglobin concentration (P Latar belakang. Suatu uji klinis acak terkontrol, dilaksanakan di Kecamatan Bilah Hulu Kabupaten Labuhan Batu pada bulan November 2006 sampai Februari 2007. Anemia defisiensi besi didiagnosis bila dijumpai Hb < 12 g/dl, MCHC <31%, RDW indeks >220, dan Mentzer indeks >13. Anak Sekolah Dasar (8 sampai 12 tahun) dengan ADB diacak untuk mendapatkan terapi besi 6 mg/kg/hari dan kelompok lain mendapat placebo selama 3 bulan. Status gizi di evaluasi dengan pengukuran antropometri sebelum dan sesudah intervensi. Hasil. Terdapat 111 anak (37,2%) menderita ADB diantara 300 anak yang diperiksa. Setelah intervensi, 108 anak menyelesaikan penelitian. Terdapat perbedaan yang bermakna pada kadar hemoglobin (P <0,05) tetapi tidak dijumpai perbedaan yang bermakna rerata berat dan tinggi badan pada kedua kelompok.. Kesimpulan. Pemberian besi tidak memberikan efek terhadap peningkatan berat dan tinggi badan.
