Background. COPD acute exacerbation and COPD stable led to an increase of airways and systemic inflammation that triggers the release of several proinflammatory cytokines into the blood circulation. This inflammatory process stimulates the liver to produce acute phase proteins such as CRP and fibrinogen. Increased levels of fibrinogen in patients with COPD showed the risk for hospitalization did increase to 70% and any increase in fibrinogen levels 1 g/l increase the risk of death 4 times in patients with COPD. Objective. To assess whether there are differences in the levels of fibrinogen in acute exacerbations of COPD and stable COPD. Materials and Methods. This is a cross sectional descriptive analytic study. Subjects admitted into the emergency room with acute exacerbations of COPD who fits the inclusion criteria were inquired for ; smoking or exposure to air pollution, previous exacerbation history, other diseases, BMI examination, leucocyte level, serum fibrinogen level during acute COPD exacerbations and stable COPD, Chest X-Ray, and spirometry. Result. From the 45 research subjects, 35 men (77.8%) and 10 women (22.2%), Based on BMI 6 subjects were underweight, 27 subjects normoweight, 12 subjects overweight, spirometry test results based on the GOLD criteria as follows: GOLD 1 1 subject (2.2%), GOLD 2 20 subjects (44.4%), GOLD 3 17 subjects (37.8%), GOLD 4 7 subjects (15.6%). Serum fibrinogen level in acute exacerbation of COPD mean and SD 426.49 ± 39.79 mg / dl compared with stable COPD with a mean and SD 283.22 ± 24.28 mg / dl significantly increased with p = 0.0001. Conclusion. Serum fibrinogen level will be elevated in acute exacerbations of COPD compared to stable COPD. Serum level of fibrinogen may be a useful biomarker to indicate the severity of COPD exacerbations.en_US
