Uji Diagnostik Red-Cell Distribution Width (RDW) terhadap Troponin T pada Kasus Sindroma Koroner Akut

Abstract

Introduction: Acute Coronary Syndrome (ACS) is used to describe the spectrum of coronary artery disease is thrombotic. Precise and rapid diagnosis can reduce morbidity and mortality. Biochemical markers of myocardial necrosis troponin T is the determinant of the most sensitive to the limitations SKA is not all hospitals have because it expensive. Red Cell Distribution Width (RDW) is one of the components of a complete blood routine examination and relatively cheap. Objective: To determine the sensitivity and specificity of RDW in detecting cases of acute coronary syndrome. Methods: One hundred and forty-three subjects research obtained from the medical records of patients hospitalized Adam Malik Hospital were collected during the period January 2014 to June 2014; ACS patients met the inclusion criteria. Complete blood laboratory data collection and troponin T. Statistical analysis with diagnostic tests to see the accuracy of the RDW. Results: Following 143 subjects showed a risk factor for age ≥ 45 years of more 126 (88.1%). Risk factors 77 subjects with hypertension (53.8%) and subjects who smoked 83 (58%) that suffered most SKA. Subjects with risk factors for diabetes mellitus and obesity, in this study is lower than non-diabetic 97 (67.8%) and obese 131 (91.6% 0). The results of diagnostic tests for the evaluation of patients with ACS RDW, sensitivity 68% and specificity 19%. Conclusion: Examination RDW levels can not be used for diagnostic tests in case SKA. Sensitivity and specificity are too low.

Pendahuluan: Sindrom Koroner Akut (SKA) digunakan untuk menggambarkan spektrum penyakit arteri koroner yang bersifat trombotik. Diagnosis yang tepat dan cepat dapat mengurangi morbiditas dan mortalitas. Petanda biokimia nekrosis miokard troponin T adalah determinan yang paling sensitive untuk SKA dengan keterbatasannya adalah tidak semua rumah sakit memiliki, mahal. Red Cell Distribution Width (RDW) merupakan salah satu dari komponen pemeriksaan rutin darah lengkap dan relative murah. Tujuan: Mengetahui nilai sensitivitas dan spesifisitas RDW dalam mendeteksi kasus sindroma koroner akut Metode: Seratus empat puluh tiga subjek penelitian yang diperoleh dari rekam medis pasien rawat inap RSUP H. Adam Malik Medan dikumpulkan selama periode Januari 2014 hingga Juni 2014; memenuhi kriteria inklusi pasien SKA. Dilakukan pendataan laboratorium darah lengkap dan troponin T. Analisa statistik dengan uji diagnostik. Hasil: Dari 143 subjek didapatkan hasil faktor risiko umur ≥ 45 tahun lebih banyak 126 (88,1%). Faktor risiko subyek dengan hipertensi 77 (53,8%) dan subyek yang merokok 83 (58%) yang paling banyak menderita SKA. Subyek dengan faktor risiko diabetes mellitus dan obesitas, pada penelitian ini lebih rendah dibandingkan yang bukan menderita diabetes 97 (67,8%) dan obesitas 131 (91,6%0). Hasil uji diagnosis RDW untuk pemeriksaan pasien SKA, sensitivitasnya 68 % dan spesifisitas 19%. Kesimpulan: Pemeriksaan kadar RDW tidak dapat digunakan untuk uji diagnostik pada kasus SKA. Sensitivitas dan spesifisitasnya terlalu rendah.
