    Hubungan Kadar Procalcitonin dengan Beratnya Pneumonia Komunitas

    Abstract
    Background The assessment of level severity in patient with Community Acquired Pneumonia (CAP) is very important determine the next management of disease. Procalcitonin (PCT) is known as one of biomarker sepsis and infection. The application of PCT is known to be used in diagnosis, to help clinician to decide antibiotic treatment and to make prognosis. It is still controversy whether the PCT early admission is related in clinical scoring system or prognostic score. Objective To determine the correlation between PCT and PSI score in CAP patients at early admission in hospital. Materials and Methods This was an cross sectional study. We had examined CAP subject with PSI and PCT. We had correlate the PCT levels with PSI to determined prognostic utiliy of PCT. Result Total CAP subjects was 60, consist of 20 subjects with severe PSI levels. From those 20 patients, one subject (1,7%) with PCT levels 0,29 ng/ml and 19 subjects (31,7%) with PCT ≥0,5 ng/ml. The biggest proportion was in low severit of PSI, about 25 subjects (41,7%) with PCT levels in 9 subjects (15%) was < 0,1 ng/ml. We had found correlation beween PSI with PCT using somers’d correlation (p=0,0001). Conclusion Procalcitonin is a biomarker of bacteremia infection that has correlation with clinical scoring system PSI. PCT use can be use to determine the prognosis in CAP admission.
     
    Latar Belakang Pada penderita pneumonia komunitas, melakukan penilaian derajat keparahan pada awal pasien masuk sangat penting sebab akan menentukkan beratnya penyakit dan rencana tata laksana selanjutnya. Saat ini procalcitonin (PCT) dikenal sebagai biomarker untuk sepsis dan infeksi. PCT dapat berperan dalam diagnosis, memutuskan pemberian antibiotik dan prognosis penderita PK Tujuan Untuk mengetahui hubungan kadar procalsitonin terhadap beratnya pneumonia komunitas yang datang ke rumah sakit. Bahan dan Cara Penelitian observasional dengan metode pengukuran cross sectional. Subjek dengan pneumonia komunitas yang masuk dari instalasi gawat darurat, setelah memenuhi kriteria dilakukan penilaian skor PSI dan PCT. Selanjutnya skor PSI dihubungkan dengan PCT dan parameter lainnya. Hasil Sebanyak 60 subjek penelitian dimana subjek yang tergolong dalam skor PSI berat sebanya 20 orang, terdiri atas 1 orang (1,7%) subjek dengan skor PSI tergolong berat memilki PCT 0,29 ng/ml dan sisanya 19 orang (31,7%) memiliki kadar PCT ≥ 0,5 ng/ml. Proporsi terbanyak didapatkan PSI ringan berjumlah 25 orang (41,7%) dengan kadar PCT terbanyak < 0,1 ng/ml sebanyak 9 orang (15%). Setelah dilakukan uji korelasi somers’d diperoleh hubungan yang signifikan antara derajat skor PSI dengan peningkatan kadar PCT (p=0,0001). Kesimpulan Procalsitonin merupakan biomarker infeksi bakteri yang memilki hubungan dengan derajat keparahan PK yang dinilai dengan skor PSI sehingga PCT dapat digunakan untuk menentukkan prognosis pasien PK sejak awal masuk ke rumah sakit.
