Pengembangan Protokol Manajemen Perawatan Luka Modern di Rumah Sakit Umum Cut Nyak Dhien Langsa

Abstract

The concept of modern wound treatment management with moisture balance has been introduced and adopted by many countriews. The benefit of this concepts of moisture balance is that it can create the environment accelerating re-epitalization, maintain moisture, will minimize infection on the moist wound-base, and can stimulate the release of the growth factor accelerating the process of wound healing. The other benefit of modern wound treatment is to minimize infection and croos-incetion, to reduce tissue of scar, to reduce time of treatment, to change bandage and to reduce cost needed. The purpose of this study was to produce the development of Standard Operational Procedure (SOP) protocol of Modern Wound Treatment Management through the application of the caring values contained as a guidance to treat a wound either acute or chronic at Cut Nyak Dhien General Hospital Langsa. That this habit of applying the values of caring in wound treatment is expected to be always applied when providing other wound treatment to other patients. The design of this action research comprised the stages of reconnaissance, planning, a acting, observing and reflecting. The data for this study were obtained through FGD, questionnaire distribution, and participant observation. The participants contributed in this study (the informants) were 12 nurses working for Cut Nyak Dhien General Hospital Langsa. The data obtained were analyzed trough quantitative on the knowledge of the nurses about modern wound treatment distributing the questionnaires on the knowledge of the nurses about modern wound treatment and caring behavior. The result of observation was analyzed through descriptive statistiec test. The result of this study showed that Standard Operational Procedure (SOP) protocol of Modern Wound Treatment Management has become the reference and used in the process of wound treatment at Cut Nyak Dhien General Hospital Langsa and it has positive impact to the improvement of knowledge, skill, and attitude of the nurses in providing nursing care, especially in terms of wound treatment. The other positive impact of the development of Standard Operational Procedure (SOP) protocol of Modern Wound Treatment Management at Cut Nyak Dhien General Hospital Langsa was that the nurses have been able to apply the value of caring when doing wound treatment. The research hopes that the management of Cut Nyak Dhien General Hospital Langsa issue the decree to determine Standard Operational Procedures (SOP) protocol of Modern Wound Treatment Management and keeps supervising when evaluating the implementation of Standadr Operational Procedure (SOP) protocol of Modern Wound Treatment Management at Cut Nyak Dhien General Hospital Langsa.

Konsep manajemen perawatan luka modern dengan basis lembab (moisture balance) telah diperkenalkan dan telah diadopsi oleh banyak negara. Keuntungan konsep lembab ini adalah membuat lingkungan yang mempercepat re-epitalisasi, menjaga kelembaban, akan menurunkan infeksi, dasar luka yang lembab dapat merangsang pengeluaran growth factor yang mempercepat proses penyembuhan luka. Keunggulan lain dari perawatan luka modern adalah mengurangi infeksi dan infeksi silang, mengurangi jaringan parut, mengurangi waktu perawatan dan mengganti balutan, serta mengurangi biaya. Penelitian ini bertujuan menghasilkan pengembangan protokol Prosedur Operasional Standar (POS) Manajemen Perawatan Luka Modern (MPLM) dengan mengaplikasikan nilai-nilai caring didalamnya sebagai pedoman dalam melakukan perawatan luka di Rumah Sakit Umum Cut Nyak Dhien (RSUCND), baik untuk luka akut, maupun luka kronik. Sehingga kebiasaan mengaplikasikan nilai-nilai caring dalam perawatan luka, diharapkan juga akan terbiasa menerapkannya pada saat memberikan asuhan keperawatan yang lain pada pasien. Desain penelitian ini adalah action research, yang terdiri-dari tahapan reconnaissance, planning, acting, observing dan reflecting. Metode pengumpulan data dilakukan melalui FGD, penyebaran kuesioner, dan observasi partisipan. Partisipan yang berkontribusi dalam penelitian ini adalah 12 orang perawat RSUCND Langsa. Analisis data menggunakan metode kuantitatif dan kualitatif. Data kualitatif diperoleh melalui hasil FGD, dan hasil observasi, sedangkan data kuantitatif yang dikumpulkan melalui penyebaran kuesioner pengetahuan perawat tentang perawatan luka modern dan pengetahuan perawat tentang prilaku caring dan hasil observasi, dianalisa dengan menggunakan uji statistik deskriptif. Penelitian ini telah menghasilkan POS MPLM yang menjadi acuan dalam melakukan perawatan luka di RSUCND, dan berdampak positif terhadap bertambahnya pengetahuan, keterampilan, dan sikap perawat dalam memberikan asuhan keperawatan, terutama dalam hal perawatan luka. Dampak lain yang didapatkan dari hasil pengembangan protokol MPLM di RSUCND adalah perawat telah mampu mengaplikasikan niali-nilai caring saat melakukan perawatan luka. Peneliti mengharapkan kepada pihak manajemen RSUCND agar membuat surat keputusan penetapan POS MPLM, dan terus melakukan supervisi untuk mengevaluasi pelaksanaan POS MPLM di RSUCND.
