Background. The prostate gland is the male organ that most often experiences enlargement, whether benign or malignant. Prostatic adenocarcinoma is the most common malignancy among malignant male urogenital systems and it is the second leading cause of death after lung cancer. BPH is the second most common disease of prostate gland disease. In Indonesia an estimated 50% prostatic adenocarcinoma and BPH occurs in men aged over 50 years. One way to detect malignancy in the prostate is by examining Prostate Specific Antigen (PSA). PSA has limitations as a diagnostic tool that increases in both benign and malignant prostate gland disease. PSA has a normal value of ≤4ng / ml. Patients who have PSA levels greater than 10 ng / ml usually suffer from malignancy in the prostate gland. Aim. To determine the relationship between PSA levels with the incidence of BPH and prostatic adenocarcinoma. Method. This research was designed using analytical research method with Cross Sectional design using retrospective data and will be analyzed to know the relationship of PSA levels with BPH and prostatic adenocarcinoma. Results. The incidence rate of prostatic adenocarcinoma and BPH in RSUP HAM during 2014-2016 period was 142 cases. Based on the incidence rate according to age range, patient with prostatic adenocarcinoma were predominantly of aged > 60 years old as many as 49 cases (34.5%) and BPH were also predominantly of aged >60 years old as many as 58 cases (49,8%). Patients with prostatic adenocarcinoma were dominated by patients with PSA> 4 as many as 64 cases (45.1%) and BPH as many as 42 cases (29.6%). Based on statistical calculation with Chi square test obtained p value of 0.000 (p <0,05). Conclusion. There was an association between PSA levels with BPH and prostatic adenocarcinoma. Latar Belakang. Kelenjar prostat adalah organ tubuh pria yang paling sering mengalami pembesaran, baik jinak maupun ganas. Adenokarsinoma Prostat merupakan keganasan paling sering terjadi diantara keganasan system urogenital pria dan menjadi penyebab kematian kedua setelah kanker paru. BPH merupakan penyakit paling sering kedua dari penyakit kelenjar prostat. Di Indonesia diperkirakan 50% Adenokarsinoma Prostat dan BPH terjadi pada pria berusia diatas 50 tahun. Salah satu cara untuk mendeteksi adanya keganasan pada prostat adalah dengan pemeriksaan Prostat Spesific Antigen (PSA). PSA memiliki keterbatasan sebagai alat diagnostic yaitu kadar PSA dapat meningkat pada penyakit kelenjar prostat yang jinak maupun ganas. PSA memiliki nilai normal ≤4ng/ml. Pasien yang memiliki kadar PSA lebih dari 10 ng/ml biasanya menderita keganasan pada kelenjar prostat. Tujuan. Untuk mengetahui hubungan antara kadar PSA dengan kejadian BPH dan Adenokarsinoma Prostat. Metode. Penelitian ini dirancang menggunakan metode penelitian analitik dengan desain Cross Sectional dengan menggunakan data retrospective dan akan dianalisa untuk mengetahui hubungan a.ntara kadar PSA dengan BPH dan Adenokarsinoma Prostat. Hasil. Angka kejadian Adenokarsinoma Prostat dan BPH di RSUP HAM pada periode 2014-2016 adalah sebanyak 142 kasus. Berdasarkan angka kejadian sesuai dengan rentang usia, penderita Adenokarsinoma Prostat didominasi kelompok usia >60 tahun yaitu sebanyak 49 kasus (34,5%) dan BPH yang didominasi kelompok usia >60 tahun yaitu sebanyak 58 kasus (49,8%). Pasien Adenokarsinoma Prostat didominasi oleh pasien dengan kadar PSA >4 yaitu 64 kasus (45,1%) dan BPH yaitu 42 kasus (29,6%). Berdasarkan perhitungan statistic dengan uji Chi square didapatkan nilai p sebesar 0,000 (p<0,05). Kesimpulan. Ditemukan adanya hubungan antara kadar PSA dengan BPH dan Adenokarsinoma Prostat.
