Background: Some studies had suggested that obesity correlate with lipid profile
disturbance. The hypothalamic-pituitary-adrenal (HPA) axis has been assumed to
be involved in obesity-associated lipid profile disturbance. The aim of this study
was to investigate the association between adrenocorticotopic hormone (ACTH)
and 11 β-Hydroxylase levels and lipid profile in young men.
Methods: This case-control study of 75 18-28 years old men which is divided in
three groups (obese, overweight, and normal) was performed at HKBP
Nommensen University Medan. ACTH, 11 β-Hydroxylase, and lipid profile (total
cholesterol, HDL cholesterol, LDL cholesterol, and triglycerides) was evaluated
in blood sample. The data was analyzed bivariately and multivariately.
Results: Lipid profile of young obese men was worse than normal men (p < 0.05).
ACTHand 11 β-Hydroxylase levels of young overweight men was higher than
normal men (p < 0.05), but there was no difference of ACTHand 11 β-
Hydroxylase levels between young obese and normal men statistically (p > 0.05).
ACTH level positively correlate with LDL cholesterol level (p= 0.039). Linear
regression testing showed that 11 β-Hydroxylase level affected total cholesterol
level (p= 0.008).
Conclusions: HPA axis activity is raised in overweight young men. ACTH level is
positively correlate with LDL cholesterol, 11 β-Hydroxylase level affected total
cholesterol level. These data shows that HPA axis play a role in lipid metabolism
disturbance in obesity and overweight young men. Latar Belakang: Beberapa penelitian telah membuktikan bahwa obesitas
berhubungan dengan gangguan profil lipid. Gangguan profil lipid pada obesitas
diduga terjadi melalui hiperaktivitas aksis hypothalamic-pituitary-adrenal (HPA).
Penelitian ini bertujuan untuk mengetahui hubungan antara adrenocorticotropine
hormone (ACTH) dan 11 β-Hydroxylase dengan profil lipid pada laki-laki usia
muda.
Metode: Penelitian kasus kontrol terhadap 75 laki-laki usia 18-28 tahun yang
dibagi menjadi tiga kelompok (obesitas, overweight, dan normal) dilakukan di
Universitas HKBP Nommensen Medan. ACTH, 11 β-Hydroxylase, dan profil
lipid (kolesterol total, HDL, LDL, dan trigliserida) diperiksa dari sampel darah
subyek penelitian.Analisis data dilakukan secara bivariat dan multivariat.
Hasil: Laki-laki usia muda yang obesitas memiliki kecenderungan profil lipid
lebih buruk dibanding dengan normal (p < 0,05). Kadar ACTH dan11 β-
Hydroxylase kelompok overweight lebih tinggi dibandingkan normal (p < 0,05),
namun kadar ACTH dan 11 β-Hydroxylase kelompok obesitas secara statistik
tidak berbeda secara signifikan dengan normal (p > 0,05). Kadar ACTH
berkorelasi positif dengan kadar kolesterol LDL (p = 0,039). Hasil uji regresi
linear menunjukkan kadar11 β-Hydroxylase berpengaruh terhadap kadar
kolesterol total (p = 0,008).
Kesimpulan: Peningkatan aktivitas aksis HPA sudah terlihat pada laki-laki usia
muda yang overweight. Kadar ACTH didapatkan berkorelasi positif dengan
kolesterol LDL, dan 11 β-Hydroxylase berpengaruh terhadap kolesterol total. Hal
ini menunjukkan bahwa aksis HPA berperan dalam gangguan metabolisme lipid
pada laki-laki usia muda obesitas dan overweight.
