dc.description.abstractBackground : Acute coronary syndrome (ACS) is one of the main problems in the cardiovascular field. The recommended revascularization procedure according to the ESC Guidelines for the management of acute myocardial infarction in patients presenting with ST-segment elevation (2017) is primary PCI. Major adverse cardiovascular events (MACE) are complications of STEMI and are an assessment of success after primary PCI. Myocardial Blush Grade (MBG) to assess determine the effectiveness of myocardial reperfusion. This study aims to determine MBG can be a predictor of MACE 6 months after primary PCI in patients with STEMI. Methods : This study was a cross-sectional study, in which 107 STEMI patients with onset <12 hours underwent the Primary PCI therapy strategy at H. Adam Malik Hospital Medan, Murni Teguh Memorial Hospital Medan and Granmed Lubuk Pakam Hospital starting October 2018 until July 2020. The results of primary PCI assessed coronary perfusion namely TIMI flow, TFC, (MBG) and evaluation of MACE for 6 months after the primary PCI. Statistical analysis was performed to assess the difference between MBG and MACE in STEMI patients undergoing Primary PCI. Result : Out of a total of 107 patients, 52 patients had MACE. There was a significant relationship between MBG ≤ 2 and 44 people (81.5%) (p<0.001) and TIMI flow ≤ 2 to 38 people (82.6%) (p<0.001). Based on the basic criteria of the research subjects on MACE, a significant relationship was found, namely the mean of onset (p 0.001) and the mean value of onset to wire (p 0.002). From the multivariate analysis of this study, there was a strong value between MBG and MACE 6 months after primary PCI with an odds ratio of 13.39. Conclusion : In this study, it was shown that a low MBG can be a predictor of MACE 6 months after primary PCI in STEMI patients with onset < 12 hours.en_US
dc.description.abstractLatar Belakang : Sindrom koroner akut (SKA) merupakan salah satu masalah utama di bidang kardiovaskular. Tindakan revaskularisasi yang direkomendasikan menurut ESC Guidelines for the management of acute myocardial infarction in patients presenting with ST-segmen elevation (2017) adalah IKP Primer. Kejadian Kardiovaskular Mayor komplikasi dari IMAEST dan merupakan penilaian dari keberhasilan setelah tindakan IKP primer . Myocardial Blush Grade (MBG) untuk menilai menentukan efektivitas reperfusi miokard. Penelitian ini bertujuan untuk mengetahui MBG dapat menjadi prediktor Kejadian Kardiovaskular Mayor (KKvM) 6 bulan paska IKP primer pada penderita IMAEST. Metode : Penelitian ini merupakan studi potong lintang, dimana 107 pasien IMAEST dengan onset < 12 jam yang menjalani strategi terapi Intervensi Koroner Perkutan (IKP) Primer di RSUP H. Adam Malik Medan, RS Murni Teguh Memorial Hospital Medan dan RS Granmed Lubuk Pakam mulai Oktober 2018. Hasil IKP primer menilai perfusi koroner yakni TIMI flow, TIMI Frame Count, Myocardial Blush Grade (MBG) dan evaluasi Kejadian Kardivaskular Mayor selama 6 bulan setelah IKP primer tersebut. Analisis statistik dilakukan untuk menilai perbedaan antara MBG terhadap KKvM pada pasien IMAEST yang menjalani IKP Primer. Hasil : Dari total 107 pasien, terdapat 52 pasien mengalami Kejadian Kardiovaskular Mayor. Hubungan signifikan MBG suboptimal terhadap KKvM 44 orang (81,5%) (p<0,001) dan TIMI flow suboptimal terhadap KKvM 38 orang (82.6%) (p<0,001). Dari kriteria dasar subyek penelitian terhadap KKvM didapati hubungan yang signifikan yaitu nilai rerata onset (p 0.001) dan nilai tengah dari onset to wire (p 0.002). Dari analisa multivariat penelitian ini didapatkan nilai yang kuat antara MBG terhadap KKvM 6 bulan paska IKP primer dengan odds ratio 13,39 Kesimpulan : Pada penelitian ini menunjukkan bahwa Myocardial Blush Grade (MBG) yang rendah dapat menjadi predictor Kejadian Kardiovaskular Mayor 6 bulan paska IKP primer pada pasien IMAEST dengan onset < 12 jam.en_US
