Hubungan Derajat Sesak Napas dan Derajat Obstruksi Saluran Napas dengan Kualitas Hidup pada Pasien PPOK Stabil di Rumah Sakit Umum Pusat Haji Adam Malik Medan

Abstract

Latar Belakang: PPOK merupakan penyakit kronis pada saluran napas yang tidak hanya mengakibatkan keluhan respirasi seperti sesak napas yang berkepanjangan dan terus menerus tetapi juga seringkali disertai dengan manifestasi dan komplikasi sistemik, seperti depresi, kecemasan, keterbatasan aktivitas dan berujung pada penurunan kualitas hidup. Tujuan: Penelitian ini bertujuan untuk mengetahui hubungan antara derajat obstruksi saluran napas dan keluhan sesak napas dengan kualitas hidup penderita PPOK Metode: Penelitian ini merupakan studi analitik dengan desain cross sectional. Pasien yang menjadi subjek penelitian merupakan penderita PPOK yang telah dikonfirmasi dengan pemeriksaan spirometri. Penilaian derajat keluhan sesak napas dilakukan dengan menggunakan skala mMRC. Derajat obstruksi ditentukan berdasarkan nilai prediksi VEP1 dari spirometri menurut kriteria GOLD. Penilaian kualitas hidup dilakukan dengan kuesioner WHO-QOL yang telah terstandarisasi. Analisis statistik dilakukan dengan uji Chi Square dimana nilai p<0.05 dinyatakan signifikan Hasil: Sebanyak 94 orang penderta PPOK dilibatkan sebagai subjek dalam penelitian ini. Sebagian besar subjek penelitian memiliki kualitas hidup buruk yaitu 52 orang (55.3%), dan 42 orang memiliki kualitas hidup buruk (44.7%). Terdapat hubungan yang signifikan antara derajat sesak napas dengan kualitas hidup pasien (p<0.001), demikian pula dijumpai hubungan yang signifikan antara derajat obstruksi saluran napas dengan kualitas hidup pasien PPOK (p<0.001) Kesimpulan: Derajat sesak napas dan derajat obstruksi saluran napas berhubungan signifikan dengan kualitas hidup pada penderita PPOK

Background: COPD is a chronic disease of the airways that not only causes respiratory complaints such as prolonged and continuous shortness of breath but is also often accompanied by systemic manifestations and complications, such as depression, anxiety, activity limitations and leads to decreased quality of life. Objective: This study aims to determine the relationship between the degree of airway obstruction and complaints of shortness of breath with the quality of life of patients with COPD Methods: This study is an analytic study with a cross sectional design. Patients who were research subjects were COPD patients who were confirmed by spirometry examination. Assessment of the degree of shortness of breath complaints was carried out using the mMRC scale. The degree of obstruction was determined based on the predicted VEP1 value from spirometry according to the GOLD criteria. The quality of life was assessed by using a standardized WHO-QOL questionnaire. Statistical analysis was performed using the Chi Square test where the p value <0.05 was significant Results: A total of 94 COPD participants were included as subjects in this study. Most of the research subjects had a poor quality of life, namely 52 people (55.3%), and 42 people had a good quality of life (44.7%). There was a significant relationship between the degree of mMRC with quality of life (p<0.001). There was a significant relationship between the degree of airway obstruction and quality of life (p<0.001). Conclusion: The degree of mMRC and the degree of airway obstruction have a significant relationship with the quality of life in patients with COPD
