dc.description.abstractBackground Chronic severe anemia is the most common disease that may increased the cardiac output when the hemoglobin level is 7g/dL for 3 months or more duration. Alteration of left ventricular function occurs frequently in chronic severe anemia children. It may be manifest as concentric LV hypertrophy, LV dilatation with or without LV hypertrophy, or systolic dysfunction. Objective To investigate the relationship between hemoglobin level and alteration of left ventricular systolic functions in children with chronic severe anemia. Method A cross-sectional study was conducted in Haji Adam Malik Hospital on October - December 2009. Consecutive samples were 6 months-15 years old chronic severe anemia children. Hematological data was collected in the beginning of the study. Left ventricular systolic functions (EF, FS) and dimensions (LVED and LVES) were detected using an echocardiography EUB 5500, Hitachi. Univariat analysis and Pearson correlation were performed. Results Thirty samples were enrolled to study. The mean of age was 113.47 months (SD 53.24). The hemoglobin values ranged from 2.1 to 6.9 g/dL. The mean of hemoglobin was 4.60 g/dL (SD 1.44). Chronic severe anemia were not associated with left ventricular systolic functions [EF 62.17% (SD 9.16); r = 0.296; P value = 0.112, FS 33.84% (SD 7.26); r = 0.115; P value = 0.545, respectively]. Chronic severe anemia were not associated with left ventricular dimensions also [LVEDD 40.25 mm (SD 6.85); r = - 0.192; P value = 0.308, LVESD 26.2 mm (SD 4.98); r = - 0.266; P value = 0.156, respectively]. Linier simple regression analysis showed no significant correlation between chronic severe anemia with left ventricular systolic functions and left ventricular dimensions. LVEDD and LVESD mean were higher compared to normal values. Conclusion There were no correlation between chronic severe anemia with the alteration of left ventricular systolic functions and left ventricular dimensions. It is also possible that chronic severe anemia is not a marker of more severe underlying alteration of left ventricular systolic functions and its dimensions.en_US
dc.description.abstractLatar belakang Anemia berat kronis merupakan penyakit tersering yang dapat mengakibatkan peningkatan cardiac output bila kadar hemoglobin di bawah atau sama dengan 7 g/dL selama 3 bulan berturut-turut atau lebih. Perubahan fungsi ventrikel kiri sering dijumpai pada anemia berat kronis. Hal ini dapat bermanifestasi sebagai hipertrofi konsentrik ventrikel kiri, dilatasi ventrikel kiri, dengan atau tanpa hipertrofi ventrikel kiri, atau disfungsi sistolik. Tujuan Untuk mengetahui hubungan antara kadar hemoglobin dan perubahan fungsi sistolik ventrikel kiri pada anemia berat kronis. Metode Suatu penelitian cross sectional dilaksanakan di RS. Haji Adam Malik Medan, pada bulan Oktober hingga Desember 2009. Sampel diambil secara consecutive sampling terdiri dari anak dengan anemia berat kronis berusia 12 bulan hingga 15 tahun. Data hematologi diambil pada awal penelitian. Fungsi sistolik ventrikel kiri (EF dan FS) dan dilatasi ventrikel kiri (LVESD dan LVEDD) diteliti dengan menggunakan ekokardiogram EUB 5500, Hitachi. Penelitian ini menggunakan analisa univariat dan Pearson Correlation. Hasil Sebanyak 30 subyek penelitian diikutsertakan pada penelitian ini. Usia rerata yaitu 113.47 bulan (SD 53.24). Kadar hemoglobin antara 2.1 sampai 6.9 g/dL. Rerata kadar hemoglobin adalah 4.60 g/dL (SD 1.44). Anemia berat kronis tidak memiliki hubungan yang bermakna dengan fungsi sistolik ventrikel kiri [EF 62.17% (SD 9.16); r = 0.296; P = 0.112, FS 33.84% (SD 7.26); r = 0.115; P = 0.545]. Anemia berat kronis tidak memiliki hubungan yang bermakna dengan dimensi ventrikel kiri [LVEDD 40.25 mm (SD 6.85); r = - 0.192; P = 0.308, LVESD 26.2 mm (SD 4.98); r = - 0.266; P = 0.156]. Penelitian ini mendapatkan rerata nilai LVEDD dan LVESD yang lebih tinggi dibandingkan nilai normal pada anemia berat kronis. Kesimpulan Penelitian ini memperlihatkan tidak adanya hubungan yang bermakna antara anemia berat kronis dengan perubahan fungsi sistolik dan dilatasi ventrikel kiri. Anemia berat kronis tidak dapat digunakan sebagai penanda telah terjadinya perubahan fungsi sistolik ventrikel kiri.en_US
