    Procalcitonin Sebagai Marker dan Hubungannya dengan Derajat Keparahan Sepsis

    Abstract
    Background : The mortality on sepsis is still high. It’s because of delayed ofthe treatment resulted from the diagnosis of sepsis estabilished more frequently imprecise. The inflammatory makers as c reactive protein and leucocyte apparently have high sensitivity and specifity where do contemn whereas blood culture examination required so long time and the result of culture often negatively. Research on Procalcitonin (PCT) formering have important of the role for the establishment diagnosis of sepsis because it’s utilized as sepsis marker and in reference to severity sepsis’degree . Objective: To determine whether procalcitonin can be used as sepsis marker and severity of sepsis. Method : Patients were assigned as 2 groups, sepsis and without sepsis with their consisting of 21 samples respectively. In the sepsis subgroup separated as sepsis only (8 samples), severe sepsis (6 samples) and sepsis shock (7 samples). All of them were examined by procalcitonin, C reactive protein, blood culture and white blood cell count. Result : The fourty two of sample were examined (21 samples for sepsis and 21 samples for without sepsis). In the sepsis group were found rate of average PCT'S and CRP rate 18,44±27,60 ng/ml and 64942,80±41199,36 mg/l, respectively. In without sepsis were found average PCT'S and CRP rate 1,33±1,50 ng/ml and 49214,28±38193,31 mg/l,respectively. The subgroup of sepsis separated as sepsis only ( 8 samples), severe sepsis ( 6 samples) and sepsis shock (7 samples) were found rate of average PCT’S 4,5±1,65, 6,34±0,74 and 44,72±36,41 ng/ml, respectively. A p value <0,05 is considered statistically significant. Conclusion : The inflammatory marker of procalcitonin can be used as sepsis marker and determined severity of sepsis. This findings showed that PCT́́́́´S rate have positively correlation with severity of sepsis. Key word : sepsis, marker, severity degree, Procalcitonin
     
    Latar belakang : Angka kematian pada sepsis masih tinggi. Hal ini dikarenakan kerterlambatan dalam penatalaksanaan oleh karena penegakan diagnose sepsis sering tidak tepat. Marker inflamasi seperti C-reactive protein dan leukosit ternyata memiliki sensitivitas dan spesifisitas yang rendah sedangkan pemeriksaan kultur membutuhkan waktu yang lama dan hasil yang didapatkan sering negative. Procalcitonin (PCT) pada penelitian-penelitian terdahulu memiliki peran yang penting dalam penegakan diagnose sepsis oleh karena dapat digunakan sebagai marker sepsis dan berhubungan dengan derajat keparahan sepsis. Tujuan : Untuk mengetahui apakah Kadar Procalcitonin dapat digunakan sebagai marker sepsis dan berhubungan dengan derajat keparahan sepsis. Metode : Pasien yang memenuhi kriteria dikelompokkan menjadi 2 kelompok, yaitu sepsis dan non sepsis dengan masing-masing terdiri dari 21 orang sampel. Khusus untuk kelompok sepsis dibagi lagi menjadi sepsis (8 sampel), sepsis berat (6 sampel) dan syok sepsis (7 sampel). Kemudian dilakukan pemeriksaan kadar Procalcitonin, C-reactive protein, kultur dan darah lengkap. Hasil : Terdapat 42 sampel yang diperiksa (21 sepsis dan 21 non sepsis). Dari kelompok sepsis secara keseluruhan didapatkan rerata kadar PCT dan CRP masing-masing 18,44±27,60 ng/ml dan 64942,80±41199,36 mg/l. Kelompok non sepsis didapatkan rerata kadar PCT dan CRP 1,33±1,50 ng/ml dan 49214,28±38193,31 mg/l secara berurutan. Pasien sepsis yang terbagi atas sepsis ( 8 orang), sepsis berat ( 6 orang) dan syok sepsis (7 orang) didapatkan rata-rata PCT 4,5±1,65 , 6,34±0,74 dan 44,72±36,41 ng/ml secara berurutan. Hal ini bermakna secara statistik, p<0,05. Kesimpulan : Kadar Procalcitonin dapat digunakan sebagai marker sepsis dan berkorelasi positif dengan derajat keparahan sepsis.
