Pengaruh Kombinasi Hemodialisis/ Hemoperfusi Terhadap Penurunan Skor Depresi pada Pasien Hemodialisis Reguler

Abstract

Background: Patients with chronic kidney disease, have a higher risk for psychological distress such as anxiety, depression and cognitive decline. Combination Hemodialysis (HD) / haemoperfusion (HP) regularly able to eliminate uremic toxins with mild-to-large molecular weight is better. HD / HP is able to remove metabolites, toxins and pathogenic factors and regulate the water, electrolyte and acid-base balance so as to improve the quality of patient's sleep and appetite also reduce itching of the skin, which in turn improve the quality and life expectancy, with lower scores and the degree of depression patients. Objective: To determine influence of combination hemodialysis/ hemoperfusion (HD/HP) against score of depression in reguler hemodialysis patients. Methods: A pre-experimental research design with one group pretest-posttest design conducted from July 2015 to September 2015 to 17 regular hemodialysis patients. Assessment using questionnaires BDI depression scores. Results: T test results showed that the average BDI score before the combination of HD / HP 18.59 ± 9 to 8.18 ± 2.83 after the combination (p <0.001). Conclusion: Combination HD / HP can lower depression scores in patients with regular HD.

Latar Belakang: Pasien dengan penyakit ginjal kronik , memiliki risiko lebih tinggi terhadap distress psikologi seperti ansietas, depresi dan penurunan kognitif . Kombinasi Hemodialisis (HD)/Hemoperfusi (HP) secara reguler mampu mengeliminasi toksin uremia dengan berat molekul ringan sampai besar secara lebih baik. HD/HP mampu menghapus metabolit, racun, dan faktor patogen serta mengatur air, elektrolit, dan keseimbangan asam basa sehingga mampu memperbaiki kualitas tidur pasien dan nafsu makan juga mengurangi gatal pada kulit, yang pada akhirnya memperbaiki kualitas dan harapan hidup, dengan menurunkan skor dan derajat depresi pasien. Tujuan: Untuk mengetahui adanya pengaruh kombinasi hemodialisis (HD) / hemoperfusi (HP) terhadap skor depresi pada pasien hemodialisis regular. Metode: Merupakan penelitian preexperimental dengan rancangan one group pre test-post test design yang dilakukan dari bulan Juli 2015 hingga September 2015 terhadap 17 pasien hemodialisis reguler. Penilaian skor depresi menggunakan kuisioner BDI. Hasil: Hasil Uji T menunjukkan bahwa rerata skor BDI sebelum kombinasi HD/HP 18,59±9 menjadi 8,18± 2,83 setelah dilakukan kombinasi (p<0,001). Kesimpulan: Kombinasi HD/HP dapat menurunkan skor depresi pada pasien HD regular.
