    Hubungan Kombinasi Hemodialisis/Hemoperfusi dengan Status Nutrisi Yang Diukur dengan Bia (Bioelectrical Impedance Analysis) pada Pasien Hemodialisis Reguler

    Abstract
    Background: Patients with Chronic Kidney Disease (CKD) undergoing regular haemodialysis often have nutritional deficiencies. The decline nutritional status in patients with CKD occur due to various factors, one of which is the high levels of urea in the blood (uremic malnutrition). Urea in the blood consists of small, medium and large molecular weight. The combination of haemodialysis with haemoperfusion (HD / HF) were able to eliminate the medium and large molecular weight. Nutritional status of patients with chronic kidney disease (CKD) can be measured by bioelectrical impedance analysis (BIA), which is a gauge body composition Objective: This study aimed to see the benefits of the combination of HD/HF for clearance medium and large weight molecular uremic toxin and to see the effects on nutritional status of patients as measured by the BIA in patients with regular haemodialysis in Medan Methods: This was a prospective cohort study from December 2013 to March 2014 on regular haemodialysis patients by comparing several BIA parameters before and after the combination of HD / HF. Results: From 20 samples were obtained males (80%) more than women (20%) with a mean age of 47.7 years old and regular haemodialysis average of 2.78 years. Obtained the mean change before and after HD/HF for IMT, RMR, FFM, the FFM (%), BCM, Glycogen, Fat, Fat (%), Protein and Mineral +0.03 respectively (p = 0.92), + 28,42 kcal (p = 0.85), + 1,64kg (p = 0.74), +2.71 (p = 0.02), + 0,58kg (p = 0.15), +16, 6kg (p = 0.05), -1,67kg (p = 0.04), -2.80 (p = 0.024), -0,21kg (p = 0.34) and -0,054kg (p = 0.42). Conclusion: Increased IMT components, RMR, FFM, the FFM (%), BCM and Glicogen showed improvement in nutritional status with a combination of HD/HF on regular hemodialysis patients.
     
    Latar Belakang: Pasien penderita Penyakit Ginjal Kronik (PGK) yang menjalani hemodialisis reguler sering mengalami gangguan nutrisi. Penurunan status gizi pada pasien PGK terjadi karena berbagai faktor, salah satunya adalah tingginya kadar ureum di dalam darah (uremic malnutrition). Ureum dalam darah terdiri atas berat molekul kecil, menengah dan besar. Kombinasi Hemodialisis/ Hemoperfusi (HD/HF) mampu mengeliminasi berat molekul menengah dan besar. Status nutrisi pasien penyakit ginjal kronis (PGK) dapat diukur dengan Bioelectrical Impedance Analysis (BIA) yang merupakan alat pengukur body composition. Tujuan: Penelitian ini bertujuan untuk melihat manfaat kombinasi HD/HF terhadap bersihan (clearance) toksin uremik berat molekul menengah dan molekul besar dengan melihat efeknya terhadap status nutrisi pasien yang diukur dengan BIA pada pasien-pasien hemodialisis reguler di Medan Metode: Jenis penelitian ini adalah cohort prospektif dari bulan Desember 2013 hingga Maret 2014 pada pasien hemodialisis reguler dengan membandingkan beberapa parameter BIA sebelum dan sesudah dilakukan kombinasi HD/HF. Hasil: Dari 20 sampel penelitian diperoleh laki-laki (80%) lebih banyak dari perempuan (20%) dengan usia rata-rata 47,7 tahun dan lama hemodialisis regular rata-rata 2,78 tahun. Didapatkan rerata perubahan sebelum dan setelah HD/HF untuk IMT, RMR, FFM, FFM (%), BCM, Glicogen, Fat, Fat (%), Protein dan Mineral secara berurutan +0,03 (p=0,92), +28,42kcal (p=0,85), +1,64kg (p=0,74), +2,71 (p=0,02), +0,58kg (p=0,15), +16,6kg (p=0,05), -1,67kg (p=0,04), -2,80 (p=0,024), -0,21kg (p=0,34) dan -0,054kg (p=0,42). Kesimpulan: Peningkatan komponen IMT, RMR, FFM, FFM (%), BCM dan Glicogen menunjukkan adanya peningkatan status nutrisi dengan kombinasi HD/HF pada pasien hemodialisis reguler.
