Pengaruh Karakteristik Individu, Iklim Kerja dan Motivasi Ekstrinsik terhadap Kelengkapan Rekam Medik dalam Manajemen Klaim Pasien Rawat Inap di Badan Layanan Umum Daerah Rumah Sakit Umum Daerah Dr. Zainoel Abidin Banda Aceh Tahun 2013

Abstract

A hospital is institution which performs a complete individual health service, and performs medical records as a mean of patient data since the whole service implementtion is recorded in them. The use of medical records has a dubbed ALFREDS (Administrative, Legal, Financial, Research, Education, Documentation, and Service). Besides that, the use of medical records as the service documentation also affects on the financial aspect since the content can be used as the material for service claim. The objective of research was to analyze the influence of individual characteristics, work atmosphere, and extrinsic motivation on completeness of document of the medical records in the management of the inpatient claims in dr. Zainoel Abidin regional general hospital Banda Aceh. The type of the research was an analytic survey with cross sectional study. Based on Chi Square test, it was found six variables which had the value of p≤0.05; they were the variables of motivation extrinsic : punishment (p=0.000), reward (p=0.000) and supervision (p=0.009), work atmosphere : work load (p=0.010), job discription (p=0.010), and co-workers (p=0.054). The result of multiple logistic regression test showed that the variable of punishment had the most significant influence on completness of document inpatients medical records , which the value of Odds Ratio of 27.830. It is recommend that the management of the hospital impose the transparent and strict punishment; otherwise, the filling out the medical records will not be accurate. This is for common interest.

Rumah sakit merupakan institusi yang menyelenggarakan pelayanan kesehatan perorangan secara paripurna dan menyelenggarakan rekam medis sebagai sarana yang memuat tentang data pasien dan semua pelaksanaan pelayanan yang tercatat di dalamnya. Kegunaan rekam medis dikenal dengan ALFREDS (Administrative, Legal, Financial, Research, Education, Documentation, and Service). Selain kegunaannya dokumentasi pelayanan juga berdampak pada aspek keuangan karena isinya dapat dijadikan bahan untuk klaim pelayanan. Tujuan penelitian adalah menganalisis pengaruh karakteristik individu, iklim kerja dan motivasi ekstrinsik terhadap kelengkapan dokumen rekam medis dalam manajemen klaim pasien rawat inap di BLUD Rumah Sakit Umum Daerah Dr.Zainoel Abidin Banda Aceh. Jenis penelitian ini adalah survey analitik dengan pendekatan cross sectional study. Berdasarkan hasil analisis terdapat 6 (enam) variabel yang mempunyai nilai p≤ 0,05 yaitu variabel motivasi ekstrinsik: sanksi (p=0,000), penghargaan (p=0,000) dan pengawasan (p=0,009), variabel iklim kerja : beban kerja (p=0,010), uraian tugas (0,010), dan variabel teman kerja (p=0,054). Hasil regresi logistic berganda sanksi merupakan variabel yang paling dominan berpengaruh terhadap kelengkapan dokumen rekam medis pasien rawat inap dengan nilai Odds Ratio 27.830. Disarankan untuk manajemen rumah sakit agar memberikan sanksi yang jelas dan tegas jika tidak melakukan pengisian rekam medis yang akurat demi kepentingan bersama.
