dc.description.abstractBackground: Sepsis is a global health problem. Several therapeutic agents continue to be developed for the management of sepsis. Ascorbic acid and thiamine are currently potential agents that are being intensively studied for their benefits in sepsis. Method: This study is a randomized controlled trial (RCT) with a pretest-posttest control group design. Randomization was done using a computer. This research was conducted at Haji Adam Malik General Hospital Medan and Grand Medistra Hospital. Blood tests were carried out at the Integrated Laboratory of the Faculty of Medicine, University of North Sumatra. This research was conducted for 14 months. The number of patients in this study was 86 septic patients were divided into 4 groups (NaCl, thiamine, ascorbic acid and combination groups). The parameters measured were the enzyme MMP-9, and the enzyme TIMP-1. Results: Of the 86 septic patients obtained Based on initial MMP-9 values, MMP- 9 levels increased in the 0.9% NaCl group (25.6%), while MMP-9 levels decreased after intervention in the thiamine, ascorbic acid and combination groups but a significant decrease in MMP-9 was found. in the ascorbic acid group (17.2%; p=0.04) and the combination (17.9%; p=0.026). Meanwhile for TIMP-1 levels, a decreasing trend was only found in the ascorbic acid (5.1%) and combination (5.9%) groups, but not significant (p >0.05). The highest MMP-9/TIMP-1 ratio were septic patients receiving thiamine with a significant linear correlation (p<0.05) between levels of MMP-9 and the ratio of MMP-9/TIMP-1 with a moderate level of correlation, and significant negative correlation (p<0.05) between TIMP-1 levels and the MMP-9/TIMP-1 ratio with a moderate level of correlation. Conclusion: Administration of thiamine alone gives better advantages compared to ascorbic acid alone and in combination.en_US
dc.description.abstractLatar belakang: Sepsis adalah masalah kesehatan global. Beberapa agen terapeutik terus dikembangkan untuk tata laksana sepsis. Asam askorbat dan tiamin saat ini merupakan agen potensial yang sedang intensif diteliti manfaatnya dalam kondisi sepsis. Metode: Penelitian ini merupakan penelitian randomized controlled trial (RCT) dengan desain pretest-posttest control group. Randomisasi dilakukan dengan menggunakan komputer. Penelitian ini dilaksanakan di RSUP Haji Adam Malik Medan dan RS Grand Medistra. Pemeriksaan darah dilakukan di Laboratorium Terpadu Fakultas Kedokteran Universitas Sumatera Utara. Penelitian ini lakukan selama 14 bulan. Pasien penelitian ini berjumlah 86 pasien sepsis yang dibagi dalam 4 grup (grup NaCl, tiamin, asam askorbat dan kombinasi). Parameter yang diukur adalah enzim enzim MMP-9, dan enzim TIMP-1. Hasil: Dari 86 pasien sepsis didapatkan berdasarkan nilai MMP-9 awal, kadar MMP-9 meningkat pada kelompok NaCl 0,9% (25,6%), sedangkan kadar MMP-9 menurun setelah intervensi pada kelompok tiamin, asam askorbat dan kombinasi namun penurunan MMP-9 yang signifikan dijumpai pada kelompok asam askorbat (17,2%; p=0,04) dan kombinasi (17,9%; p=0,026). Sementara untuk kadar TIMP- 1, kecenderungan penurunan hanya dijumpai pada kelompok asam askorbat (5,1%) dan kombinasi (5,9%), tetapi tidak signifikan (p >0,05). Rasio MMP-9/TIMP-1 yang paling besar adalah pasien sepsis yang mendapat tiamin dengan korelasi linier yang bermakna (p<0,05) antara kadar MMP-9 dan rasio MMP-9/TIMP-1 dengan tingkat korelasi sedang, dan korelasi negatif yang bermakna (p<0,05) antara kadar TIMP-1 dan rasio MMP-9/TIMP-1 dengan tingkat korelasi sedang. Kesimpulan: Pemberian tiamin tunggal memberikan keuntungan lebih baik dibandingkan dengan asam askorbat tunggal dan kombinasi.en_US
