Evaluasi Penerapan Parameter Rule of Tens sebagai Kriteria Standar Penatalaksanaan Celah Bbir dengan Labioplasty di RSU Mitra Sejati Medan Tahun 2020-2021

Abstract

Cleft lip is the most common congenital orofacial abnormality that is formed due to failure of the medial nasal process to fuse with the maxillary process between 4th and 6th week of intrauterine life (IUL). A surgical procedure, called labioplasty, is required to close the cleft and reconstruct the lip. The standard criteria used in determining the right time to do the labioplasty is the Rule of Tens. This study aimed to evaluate the application of the Rule of Tens parameters, including body weight, age, hemoglobin level, and leukocyte count as a standard criteria for labioplasty at Mitra Sejati General Hospital Medan in 2020-2021. This research is a descriptive research using survey method. This research results on evaluating the application of the Rule of Tens, that there were 31 patients (32.6%) who applied the Rule of Tens and 64 patients (67.4%) who did not apply the Rule of Tens. Patients with body weight ³ 10 pounds (4.5 kg) amounted to 95 patients (100%) and there were no patients with body weight < 10 pounds (4.5 kg). Patients who aged ³ 10 weeks (3 months) amounted to 82 patients (86.3%) and patients aged < 10 weeks (3 months) amounted to 13 patients (13.7%). Patients with hemoglobin level ³ 10 g/dL amounted to 84 patients (88.4%) and patients with hemoglobin level < 10 g/dL amounted to 11 people (11.6%). Patients with leukocyte count <10,000 cells/μL amounted to 43 patients (45.2%) and patients with leukocyte count >10,000 cells/μL amounted to 52 patients (54.7%). This study concludes that the Rule of Tens was not strictly applied at Mitra Sejati General Hospital.

Cleft lip atau celah bibir merupakan kelainan kongenital orofasial yang paling umum ditemukan yang terbentuk karena kegagalan fusi prosesus nasal medial dengan prosesus maksilaris pada minggu ke-4 sampai minggu ke-6 intrauterin life (IUL). Untuk menutup celah dan merekonstruksi bibir, diperlukan tindakan secara bedah yaitu labioplasty. Kriteria standar yang digunakan dalam menentukan waktu yang tepat untuk dilakukan labioplasty adalah Rule of Tens. Tujuan penelitian ini adalah untuk mengevaluasi penerapan parameter Rule of Tens, meliputi bedat badan, usia, kadar hemoglobin, dan jumlah leukosit sebagai kriteria standar labioplasty di RSU Mitra Sejati Medan Tahun 2020-2021. Penelitian ini merupakan penelitian deskriptif dengan metode survei. Hasil penelitian evaluasi penerapan Rule of Tens, pasien yang menerapkan Rule of Tens berjumlah 31 orang (32,6%) dan pasien yang tidak menerapkan Rule of Tens berjumlah 64 orang (67,4%). Pasien dengan berat badan ³ 10 pon (4,5 kg) berjumlah 95 orang (100%) dan tidak ditemukan pasien dengan berat badan < 10 pon (4,5 kg). Pasien dengan usia ³ 10 minggu (3 bulan) berjumlah 82 orang (86,3%) dan pasien dengan usia < 10 minggu (3 bulan) berjumlah 13 orang (13,7%). Pasien dengan kadar hemoglobin ³ 10 g/dL berjumlah 84 orang (88,4%) dan pasien dengan kadar hemoglobin <10 g/dL berjumlah 11 orang (11,6%). Pasien dengan jumlah leukosit < 10.000 sel/μL berjumlah 43 orang (45,2%) dan pasien dengan jumlah leukosit > 10.000 sel/μL berjumlah 52 orang (54,7%). Kesimpulan penelitian ini adalah Rule of Tens tidak diterapkan secara baku di RSU Mitra Sejati Medan.
