    Pengaruh Intervensi Farmasis Terhadap Drug Related Problems (DRPs), Outcome Terapi, Kepatuhan dan Kualitas Hidup Pasien Diabetes Mellitus Tipe 2 Rawat Jalan

    Abstract
    The Drug-Related Problems (DRPs) in the treatment of Diabetes Mellitus Type 2 (DMT2) are drug-related problems that can affect the outcome of therapy, patient compliance in taking medication, and the patient's quality of life. Pharmacist intervention in T2DM patients is the right strategy in overcoming the incidence of DPRs, preventing complications, and reducing morbidity and mortality. The purpose of this study was to identify and evaluate the incidence of DRPs in outpatient DMT2 patients and analyze the effect of the intervention on the number of DRPs incidences, the clinical outcome of KGD, adherence level, and quality of life of T2DM patients before and after the intervention. This retrospective-prospective cohort-analytic study analyzed the incidence of DRPs, therapy outcomes, adherence, and quality of life before and after the education of outpatients with DMT2 (n=48) at RSUD Dr. Tengku Mansyur Tanjungbalai City for the period December 2020 - May 2021. Identification of DRPs events using the European Pharmaceutical Care Network (PCNE) V9.0 instrument and reliable references. Quality of life of patients using the EQ-5D-3L questionnaire, and patient compliance data obtained using the MMAS-8 questionnaire and Blood Sugar Level (BSL) scores. The data were analyzed using the Wilcoxon Signed-Rank test on the SPSS Program. The results showed that the average incidence of DRPs before the intervention was 1.041 ± 0.041 and decreased after the intervention by the pharmacist to 0.104 ± 0.061. The mean BSL before the intervention was 262.02±5.91 and decreased after the pharmacist intervention was 168.604±3.07. The mean MMAS-8 adherence score before intervention was 4.833 ± 0.08 and increased after the pharmacist intervention became 7.793 ± 0.059. The mean quality of life score of EQ-5D-3L before intervention was 0.719 ± 0.005 and increased after the pharmacist intervention to 0.977 ± 0.009. Based on the results of the study, it was concluded that pharmacist intervention could reduce the incidence of DRPs, improve therapeutic outcomes, adherence levels, and quality of life of outpatients with DMT2.
     
    Drug Related Problems (DRPs) pada pengobatan Diabetes Melitus Tipe 2 (DMT2) merupakakan masalah terkait obat yang dapat mempengaruhi outcome terapi, kepatuhan pasien dalam minum obat, serta kualitas hidup pasien. Intervensi farmasis pada pasien DMT2 adalah strategi yang tepat dalam penanggulangan kejadian DPRs , mencegah terjadinya komplikasi, serta menurunkan angka morbiditas dan mortalitas. Tujuan penelitian ini adalah untuk mengidentifikasi dan mengevaluasi kejadian DRPs pada pasien DMT2 rawat jalan serta menganalisis pengaruh intervensi terhadap jumlah kejadian DRPs, outcome klinis KGD, tingkat kepatuhan dan kualitas hidup pasien DMT2 sebelum dan sesudah intervensi. Penelitian analitik retrospektif-prospektif cohort-studi ini menganalisis kejadian DRPs, outcome terapi, kepatuhan, dan kualitas hidup sebelum dan sesudah edukasi terhadap pasien DMT2 rawat jalan (n=48) di RSUD Dr. Tengku Mansyur Kota Tanjungbalai periode Desember 2020 - Mei 2021. Identifikasi kejadian DRPs menggunakan instrumen pharmaceutical care network europe (PCNE) V9.0 dan referensi yang dapat dipercaya. Kualitas hidup pasien menggunakan kuesiner EQ- 5D-3L, dan data kepatuhan pasien diperoleh menggunakan kuesioner MMAS-8 dan nilai KGD. Data dianalisis menggunakan uji Wilcoxon Signed Rank pada Program SPSS. Hasil penelitian menunjukkan kejadian DRPs rerata sebelum intervensi 1,041 ± 0,041 dan menurun setelah adanya intervensi farmasis menjadi 0,104 ± 0,061. KGD sewaktu rata-rata sebelum intervensi 262,02±5,91 dan menurun setelah adanya intervensi farmasis menjadi 168,604±3,07. Skor kepatuhan MMAS-8 rata- rata sebelum intervensi 4,833 ± 0,08 dan meningkat setelah adanya intervensi farmasis menjadi 7,793 ± 0,059. Skor kualitas hidup EQ-5D-3L rata-rata sebelum intervensi 0,719 ± 0,005 dan meningkat setelah adanya intervensi farmasis menjadi 0,977 ± 0, 009. Berdasarkan hasil penelitian disimpulkan bahwa intervensi farmasis dapat menurunkan angka kejadian DRPs, meningkatkan outcome terapi, tingkat kepatuhan, dan kualitas hidup pasien DMT2 rawat jalan.
