    Pengaruh Bahan dan Desain Koping Marjin Collarless terhadap Adaptasi Marjinal Mahkota Logam Porselen

    Abstract
    Porcelain fused to metal have been considered the gold standard in prosthodontics for the past 40 years. Porcelain fused to metal are still commonly used because of clinically satisfactory mechanical, aesthetic, marginal and internal adaptability properties and lower cost. However, in some cases, the labio marginal metal collar is not aesthetically acceptable to the patient. This can be overcome by a marginal porcelain butt joint coping design at the labio marginal. Recent clinical studies have introduced a type of cobalt chromium coping material that is biologically more acceptable than nickel chromium which many people have allergic reactions. In this study, Ni-Cr and Co-Cr coping materials were used with full metal collarless and modified metal collarless coping designs. The purpose of this study was to determine the effect of materials and coping design on the marginal adaptation of porcelain fused to metal. This type of research is experimental laboratory. The typodontic central incisor was prepared and duplicated using a CAD / CAM scan to become zirconia and implanted in an acrylic resin beam for the manufacture of 24 porcelain metal samples. The application of an opaque layer on both coping designs were burned at a temperature of 975˚C followed by the application of dentin, enamel and glazing. The measurement of the marginal adaptation value in the porcelain fused to metal sample group was carried out using a Stereomicroscope (Olympus SZX 10) which was computerized using Olympus Stream Software in micro metres (μm) units. The sample is placed under a stereomicroscope at five points where the marginal gap will be calculated. Statistical data using Independent T-Test showed a significant difference with the mean value of marginal gap and standard deviation of Ni-Cr coping material with full metal collarless coping design (119.76 ± 25.8 )μm, and Co-Cr coping material was (90.03 ± 12.03) μm. The mean value of the marginal gap and standard deviation of the Ni-Cr coping material with the modified metal collarless coping design was (87.82 ± 23.36) μm, and the Co- Cr coping material was (59.41 ± 11.48) μm. The results of the Independent T-Test showed that there was a significant difference between Ni-Cr and Co-Cr coping materials with a full metal collarless coping design on the marginal adaptation of procelain fused to metals with p value = 0.029 (p <0.05). Significant difference of Ni- Cr and Co-Cr coping materials with modified metal collarless coping design on the marginal adaptation of procelain fused to metals with p value = 0.023 (p <0.05). Significant difference in the design of full metal collarless and modified metal collarless coping with Ni-Cr coping material on the marginal adaptation of procelain fused to metals with p value = 0.049 (p <0.05). Significant differences in the design of full metal collarless and modified metal collarless coping with Co-Cr coping material on the marginal adaptation of procelain fused to metal with p value = 0.001 (p <0.05). The results of the study concluded that the Co-Cr coping material and the modified metal collarless coping design have best marginal adaptation and are clinically acceptable, so this design can be recommended for clinical application in cases that require maximum esthetics.
     
    Mahkota logam porselen telah dianggap sebagai standar emas pada bidang prostodonsia selama 40 tahun terakhir. Mahkota logam porselen masih banyak digunakan karena memiliki sifat mekanik, estetik, adaptasi marjinal dan internal yang memuaskan secara klinis dan biaya yang lebih murah. Namun, pada beberapa kasus, metal collar pada labio marjinal secara estetik kurang dapat diterima oleh pasien. Hal ini dapat diatasi dengan desain koping marjinal porcelain butt joint pada labiomarjinal. Studi klinis baru-baru ini telah memperkenalkan jenis bahan koping cobalt chromium yang secara biologis lebih ramah dibandingkan dengan nickel chromium yang dapat menimbulkan reaksi alergi. Dalam penelitian ini digunakan bahan koping Ni-Cr dan Co-Cr dengan desain koping full metal collarless dan modified metal collarless. Tujuan dari penelitian ini adalah untuk mengetahui pengaruh bahan dan desain koping terhadap adaptasi marjinal mahkota logam porselen. Jenis penelitian ini adalah eksperimental laboratoris. Gigi insisivus sentralis typodont dipreparasi dan diduplikasikan menggunakan CAD / CAM sehingga menjadi dai zirkonia dan ditanam di dalam balok resin akrilik untuk pembuatan 24 sampel logam porselen. Aplikasi lapisan opak pada kedua desain koping yang di bakar pada suhu 975˚C yang dilanjutkan dengan aplikasi dentin, enamel dan glazing. Pengukuran nilai adaptasi marjinal pada kelompok sampel logam porselen dilakukan dengan alat Stereomikroskop (Olympus SZX 10) yang terdata secara komputerisasi menggunakan Software Olympus Stream dalam satuan mikro meter (μm). Sampel diletakkan di bawah stereomikroskop pada lima titik referensi yang akan dilakukan perhitungan besarnya celah marjinal. Data statistik dengan T-Test Independent terlihat perbedaan yang signifikan dengan nilai rerata celah marjinal dan standar deviasi pada bahan koping Ni-Cr dengan desain koping full metal collarless adalah (119,76 ± 25,8 ) μm, dan bahan koping Co-Cr adalah (90,03 ± 12,03) μm. Nilai rerata celah marjinal dan standar deviasi pada bahan koping Ni-Cr dengan desain koping modified metal collarless adalah (87,82 ± 23,36) μm, dan bahan koping Co-Cr adalah (59,41 ± 11,48) μm. Hasil penelitian dapat disimpulkan bahwa bahan koping Co-Cr dengan desain koping modified metal collarless memiliki adaptasi marjinal yang paling baik dan dapat diterima secara klinis, sehingga desain ini dapat direkomendasikan untuk aplikasi klinis pada kasus-kasus yang memerlukan estetis maksimal.
