    Pengaruh Bahan dan Desain Koping Marjin Collarless terhadap Ketahanan Fraktur Mahkota Logam Porselen

    Abstract
    Porcelain fused to metal (PFM) is currently the most popular restoration that can be used in the anterior teeth. However the coping collar design on facial margin gingiva caused dark and shadowed zone in gingival, that was named Umbrella Effect and it would be a patient complaint because it is not aesthetically pleasing. This can be overcome with a collarless design. One of the most aesthetically pleasing collarless designs is the modified metal collarless, but this coping design has the disadvantage of reduced fracture resistance. Recent clinical studies have introduced the use of Co-Cr alloy instead of Ni-Cr alloy as a coping material because it has better physical and mechanical properties so as to increase the fracture resistance of PFM. In this study, Ni-Cr and Co-Cr coping materials were used with full metal collarless and modified metal collarless coping designs. The purpose of this study was to determine the effect of materials and coping designs on the fracture resistance of porcelain metal crowns. The typodontic central incisor teeth were preparation and duplicated from autopolymerized acrylic resin and then casted into metal as much as 24 samples. The samples were divided into 4 groups with group A: Ni-Cr with full metal collarless design, group B: Ni-Cr with modified metal collarless design, group C: Co-Cr with full metal collarless design and group D: Co-Cr with design modified metal collarless. The measurement of the fracture resistance value was carried out with a universal testing machine (Torsee UTM AMU-10). The sample is placed at an angle of 450 from the length of the tooth axis and the tip of the pressure is placed 2 mm below the incisal from the palatal surface. Statistical data using Independent T-Test showed a significant effect with the mean value of fracture resistance and standard deviation of Ni-Cr coping material (867,68 ± 187.68)N, and Co-Cr coping material was (1775,89 ± 470.67)N with full metal collarless coping design. Significant effect with the mean value of fracture resistance and standard deviation of Ni-Cr coping material (753,62 ± 125,97)N, and Co-Cr coping material was (1152,62 ± 218,14)N with modified metal collarless coping design. There is no significant effect with the mean value of fracture resistance and standard deviation of full metal collarless coping design (867,68 ± 187,68)N, and modified metal collarless coping design (753,62 ± 125,97)N with Ni-Cr coping material. Significant effect with the mean value of fracture resistance and standard deviation of full metal collarless coping design (1775,89 ± 470,67)N, and modified metal collarless coping design (1152,62 ± 218,14)N with Co-Cr coping material. The results of the study concluded that the Co-Cr coping material with the modified metal collarless coping design still has good fracture resistance and is still above the maximum biting force of human incisor and is clinically acceptable, so this design can be recommended for clinical applications in cases that require maximum aesthetics.
     
    Porcelain fused to metal (PFM) masih sangat populer penggunaannya khususnya untuk gigi anterior. Tetapi desain koping collar pada tepi ginggiva fasial menyebabkan ginggiva berwarna dan berbayang kehitaman yang disebut dengan Umbrella Effect dan akan menjadi keluhan pasien karena tidak estetis. Hal ini dapat diatasi dengan desain collarless. Salah satu desain collarless yang paling baik dari segi estetis adalah modified metal collarless tetapi desain koping ini memiliki kelemahan yakni ketahanan fraktur yang berkurang. Dalam penelitian digunakan bahan koping Ni-Cr dan Co-Cr dengan desain koping full metal collarless dan modified metal collarless. Studi klinis baru-baru ini telah memperkenalkan penggunaan aloi Co-Cr menggantikan aloi Ni-Cr sebagai bahan koping karena mempunyai sifat fisik dan mekanis yang lebih baik sehingga dapat meningkatkan ketahanan fraktur dari PFM. Tujuan penelitian ini adalah untuk mengetahui pengaruh bahan dan desain koping terhadap ketahanan fraktur mahkota logam porselen. Jenis penelitian ini adalah eksperimental laboratoris. Gigi insisivus sentralis typodont dipreparasi dan diduplikasi dai resin menggunakan resin akrilik autopolimerisasi kemudian di casting menjadi dai logam sebanyak 24 sampel. Sampel dibagi menjadi 4 kelompok dengan kelompok A koping Ni-Cr dengan desain full metal collarless, kelompok B koping Ni-Cr dengan desain modified metal collarless, kelompok C koping Co-Cr dengan desain full metal collarless dan kelompok D koping Co-Cr dengan desain modified metal collarless. Pengukuran nilai ketahanan fraktur dilakukan dengan universal testing machine (Torsee UTM AMU- 10). Sampel diletakkan pada sudut 45o dari panjang aksis gigi dan ujung penekan diletakkan 2 mm dibawah insisal dari permukaan palatal. Analisis statistik menggunakan T-Test Independent terlihat pengaruh yang signifikan dengan nilai rerata ketahanan fraktur dan standar deviasi pada bahan koping Ni-Cr (867,68 ± 187.68) N dan Co-Cr (1775,89 ± 470.67) N dengan desain koping full metal collarless, p = 0,004 (p<0,05). Terdapat pengaruh yang signifikan pada nilai rerata ketahanan fraktur dan standar deviasi pada bahan koping Ni-Cr (753,62 ± 125,97) N, dan Co-Cr (1152,62 ± 218,14) N dengan desain koping modified metal collarless, p = 0,003 (p<0,05). Tidak terdapat pengaruh signifikan pada nilai rerata ketahanan fraktur desain koping full metal collarless (867,68 ± 187,68 )N dan modified metal collarless (753,62 ± 125,97) N dengan bahan koping Ni-Cr, p = 0,245 (p>0,05). Terdapat pengaruh signifikan pada nilai rerata ketahanan fraktur desain koping full metal collarless (1775,89 ± 470,67)N dan modified metal collarless (1152,62 ± 218,14) N dengan bahan koping Co-Cr, p = 0,015 (p<0,05). Hasil penelitian dapat disimpulkan bahwa bahan koping Co-Cr dengan desain koping modified metal collarless masih memiliki ketahanan fraktur yang baik serta masih diatas nilai maksimum kekuatan gigit insisivus manusia dan dapat diterima klinis, sehingga desain ini dapat direkomendasikan untuk aplikasi klinis pada kasus – kasus yang memerlukan estetis maksimal.
