    Potensi Antikanker Ekstrak Kulit Kayu Manis (Cinnamomum Burmannii) Dibandingkan dengan Cisplatin terhadap Biomarker Apoptosis, P21, Cyclin E, Ros, Cytochrome C, Caspase-3, P-Glycoprotein dan Ctr1 pada Kultur Sel Kanker Ovarium SKOV3

    Abstract
    Background: In Indonesia, ovarian cancer is the third most common cancer after breast and cervical cancer. Chemotherapy resistance is responsible for recurrency in ovarian cancer and mortality. Cinnamon bark extract has potential effect as anticancer and prevention of chemotherapy cisplatin resistancy. Aim: To prove cinnamon bark (Cinnamomum burmannii) extract alone and in combination with cisplatin has potency as anticancer and prevention of chemotherapy cisplatin resistancy. Methods: This study is exprement and explorative study with cinnamon bark extract alone and in combined with cisplatin in ovarian cancer cell culture SKOV3. This study was performed at Pharmacognosy Laboratory of the Faculty of Pharmacy, University Sumatera Utara, Stem Cells and Tissues Engineering Research Cluster (SCTE) laboratory of IMERI Universitas Indonesia, and FKUI Integrated Laboratory from September 2020-November 2021. This study was carried out to see effect of cinnamon bark extract and or combination with cisplatin toward apoptosis biomarkers; p21, cyclin E, cytochrome C, ROS, caspase-3, p glycoprotein and CTR1 with flow cytometry. Four groups were used; combination 1 (1xIC50 cinnamon bark and 1xIC50 cisplatin), combination 2 (¾x cinnamon bark and ¾ x cisplatin), combination 3 (½x cinnamon bark and ½x cisplatin), and combination 4 (¼x cinnamon bark and ¼x cisplatin). Results: The results are from MTS assay IC50 cinnamon bark extract was 68,73 µg/ml and cisplatin were 117,5 µM. The combination 1 have cytotoxic effect with viability cell 40.58%. From flowcytometry result that cinnamon bark extract and or combination cisplatin had aceleration apoptosis with increase of annexin V expression for 25.63 ± 1.98 and 42.77 ± 2.45, increased of cytochrome c expression for 19.73 ± 0.95 and 42.37 ± 0.31, increased of caspase3 for 9.27 ± 0.06 and 20.3 ± 0.26, increase of p21 expression for 9.77 ± 0.38 and 40.5 ± 0.53, decrease of cyclin e expression 11.4 ± 1.20 and 1.2 ± 0, decrease of p-glycoprotein for 12 ± 0.7 and 1.2 ± 0, increase of CTR1 for 19.73 ± 0.49 and 12 ± 0.92, and decrease of ROS 1,43 ± 0,15 and 0.3 ± 0.1. Conclusion: Cinnamon bark extract alone and in combination with cisplatin have potential effect anticancer and prevention of cisplatin resistant at ovarian cancer cell culture SKOV3.
     
    Latar Belakang: Di Indonesia, kanker ovarium merupakan kanker ketiga terbanyak setelah kanker payudara dan serviks. Resistensi kemoterapi cisplatin bertanggung jawab terhadap terjadinya rekurensi pada kanker ovarium dan tingginya morbiditas dan mortalitas. Ekstrak kulit kayu manis (Cinnamomum burmannii) berpotensi sebagai antikanker dan pencegahan resistensi kemoterapi cisplatin. Tujuan: Untuk membuktikan potensi kulit kayu manis (Cinnamomum burmannii) tunggal maupun kombinasi dengan cisplatin sebagai antikanker dan pencegahan resistensi kemoterapi cisplatin. Metode: Penelitian ini merupakan penelitian eksperimental, eksploratif laboratorium. Dilakukan pada Laboratorium Farmakognosi Fakultas Farmasi Universitas Sumatera Utara, Laboratorium Stem Cells and Tissues Engineering Research Cluster (SCTE) IMERI FKUI, dan Laboratorium Terpadu FKUI pada bulan September 2020 sampai November 2021. Untuk melihat pengaruh ekstrak kulit kayu manis tunggal dan kombinasi dengan cisplatin pada sel kultur kanker ovarium SKOV3 terhadap biomarker apoptosis, p21, cyclin E, cytochrome C, ROS, caspase-3, CTR1, dan pglycoprotein, dengan flow sitometri. Setelah di dapati IC50 ekstrak kulit kayu manis dan IC50 cisplatin, dilanjutkan dengan IC50 empat kelompok kombinasi yaitu kombinasi 1 (1xIC50 kulit kayu manis dan 1xIC50 cisplatin), kombinasi 2 (¾ x IC50 ekstrak kulit kayu manis dan ¾ x IC50 cisplatin), kombinasi 3 (½ IC50 ekstrak kulit kayu manis dan ½ x IC50 cisplatin) dan kombinasi 4 (¼x IC50 ekstrak kulit kayu manis dan ¼x cisplatin). Hasil: Dari penelitian ini pada uji MTS didapatkan IC50 ekstrak kulit kayu manis 68,73 µg/ml dan cisplatin sebesar 117,5 µM. Kelompok kombinasi 1 memiliki efek sitotoksik dengan viabilitas sel 40.58%. Dari hasil flowsitometri didapati ekstrak kulit kayumanis tunggal dan kelompok kombinasi1 mengakselerasi apoptosis dengan peningkatan annexin V sebesar 25,63 ± 1,98 dan 42,77 ± 2,45; peningkatan cytochrome C sebesar 19,73 ± 0,95 dan 42,37 ± 0,31 ; peningkatan caspase3 sebesar 9,27 ± 0,06 dan 20,3 ± 0,26 ; peningkatan p21 sebesar 9,77 ± 0,38 dan 40.5 ± 0.53; penurunan cyclin e sebesar 11,4 ± 1,20 dan 1,20 ± 0; penurunan p-glycoprotein sebesar 12 ± 0,7 dan 1,2 ± 0,1; peningkatan CTR1 sebesar 19,73 ± 0,49 dan 12 ± 0,92, dan menurunkan ROS sebesar 1,43 ± 0,15 dan 0,3 ± 0,1. Kesimpulan: Ekstrak kulit kayu manis tunggal dan kombinasi dengan cisplatin berpotensi sebagai antikanker dan pencegahan resistensi cisplatin pada kultur sel ovarium SKOV3.
