    Abstract
    The high maternal mortality rate in Indonesia is related to the low result of the implementation of Ante Natal Care (ANC). In the city of Subulussalam, the result of the program reported is still far compared to the target decided, namely, first visit (K1) reported was 72.96% and fourth visit (K4) reported was 65.56% compared to the national target of 91% in 2007. The result of K1 and K4 programs reported still need to be increased as optimum as possible in order that the target of the K1 and K4 programs (95%) to be achieved in 2010 can be materialized. Based on the previously mentioned condition, a study of the influence of the geographical factor and the behavior of pregnant mothers on the low result of Ante Natal Care program (K1 and K4) in Puskesmas (Community Health Center) Runding, the city of Subulussalam needs to be conducted. The purpose of this explanatory study is to analyze the influence of geographical factor and behavior of pregnant mothers to the low result of reported Ante Natal Care (K1 and K4) program in Puskesmas Runding, the city of Subulussalam. The samples for this study are 98 pregnant mothers who were selected through simple random sampling technique. The data obtained were analyzed through multiple regression tests. The result of this study shows that all (100%) of the Ante Natal Care for the pregnant mothers were implemented by health workers, but the K1 and K4 reported is still low, only 69.4%, compared to the national target (K1 = 92.2% and K4 = 90%). Most of the distance from the respondents’ home to the health service location (63.3%) is < 5 km (near) which can be reached in < 15 minutes (63.3%), 60.2% reached the health service location by public transportation. In terms of the factor of behavior (knowledge, attitude and action) in pregnancy examination, it is found out that knowledge is in good category (52.0%), attitude is in good category (63.3%), and action is also in good category (73.5%). Statistically, all of the geographical factors (distance, time taken, and transportation) and the factor of behavior (knowledge, attitude, and action) have an influence to the Ante Natal Care (p < 0.05). It is expected that Subulussalam Health Service through Puskesmas Runding to improve the health extension on the importance of Ante Natal Care for the pregnant mothers as an attempt to minimize the maternal mortality rate and increase the participation of health workers in doing Ante Natal Care, especially in the remote villages and the areas where the facilities for Ante Natal Care are not available.
