Background : Contrast-Induced Nephropathy (CIN) is an important complication of
procedures involving contrast media eg, coronary angiography in patients with acute
coronary syndromes (ACS). It has been recognized for some time that the development of
CIN is strongly associated with increasing risk of in-hospital and long-term morbidity
and mortality. The aim of this study was to examine the association between ratio of the
volume of contrast media to the creatinine clearance (V/CrCl) in patients with acute
coronary syndromes undergoing percutaneous coronary intervention (PCT).
Methods : This is a mix cohort study involving 76 patients with ACS undergoing PCI
after admitted to CVCU and RIC Adam Malik Hospital Medan on January-June and
October-December 2015. The CIN was defined as an increase of creatinine serum 2.25%
from baseline or an absolute increase 2:0 .5 mg/dl at 24 h after PCI.
Results : Of the 76 patients, the incidence of CIN occurred in 18 (23.7%) patients. The
group that experienced the incidence of CIN have an older mean age (60.4 vs 53.5 yr, p =
0.003), lower Hb ievel (13.4 vs 14.3 mg/dl, p = 0.025), higher blood glucose level on
admission (212.1 vs 152.7 mg/dl, p = 0.001), and higher GRACE and Mehran scores
(159.7 ± 29.9, p = 0.001; 5.7 ± 2.8, p = 0.005). Ratio of the volume of contrast media to
creatinine clearance between two groups was not different statistically.
Conclusions : There was no correlations between ratio of the volume of contrast media
to the creatinine clearance with incidence of CIN during hospitalization in patients with
ACS undergoing PCI.
