Perubahan Struktur Seluler Retina pada Fakoemulsifikasi: Studi Berbasis Spectral Domain Optical Coherence Tomography (SD OCT)

tomography (SD OCT) examinations.
Methods: Twenty seven eyes of 27 patients underwent phacoemulsification
were studied. All 9 areas and volume of macula were examined by SD OCT
pre operatively and 2 months post operatively. Effective Phaco Time (EPT)
and Absolute Phaco Time (APT) were also recorded.
Results: There are statistically significant differences in macular thickness
between pre and post SD OCT examinations on 3 areas including macular
volume. On temporal paracentral area, preoperative and postoperative
macular thicknesses were 238.26 ± 83.95 μm and 291.11 ± 42.12 μm
consecutively (p=0.001). On temporal peripheral area, preoperative and
postoperative macular thicknesses were 180.81 ± 97.37 μm and 252.15 ±
31.80 μm respectively (p=0.000). On superior peripheral area, preoperative
and postoperative macular thicknesses were 227.19 ± 69.81 μm and 269.48
± 25.60 μm sequentially (p=0.005). There were significant changes in
preoperative and postoperative macular volumes were 7.85 ± 1.78 μm3 and
9.46 ± 1.05 μm3 successively (p=0.000). Correlations between APT/EPT and
macular cellular structures (p>0.01) was ·not presented significantly. Better
visual acuity (p=0.000), and IOP lowering (p=0.000) were also achieved
significantly in this study.
Conclusion: Macular thickness changes in areas of temporal paracentral,
temporal peripheral, superior peripheral and also macular volume may be
used as new biomarkers to measure the effects of IOP fluctuations and
maneuvers in the intraocular surgeries. From this study, there were no
correlations between APT/EPT and macular thickness changes in
phacoemulsification. Phacoemulsification helps improve visual acuity and
IOP lowering and may be safe for the retinal cellular structures. To ensure
sight for a lifetime, SD OCT examinations before and after
phacoemulsification may be compulsory. Latar Belakang: Untuk mengetahui pengaruh fakoemulsifikasi pada
ketebalan dan volume makula menggunakan pemeriksaan Spectral Domain
Optical Coherence Tomography (SD OCT)
Metode: Dua puluh tujuh mata dari 27 pasien yang menjalani
fakoemulsifikasi dipelajari dan diteliti. Seluruh 9 area dan volume macula
diperiksa dengan SD OCT sebelum operasi dan 2 bulan setelah operasi.
Effective Phaco Time (EPT) dan Absolute Phaco Time (APT) juga dicatat.
Hasil: Ada perbedaan signifikan secara statistik pemeriksaan SD OCT
terhadap ketebalan makula antara sebelum dan sesudah fakoemulsifikasi
pada 3 area dan volume makula. Pada area parasentral temporal ketebalan
makula sebelum operasi adalah 238.26 ± 83.95 μm dan setelah operasi
291.11 ± 42.12 μm (p=0.001). Pada area perifer temporal ketebalan makula
sebelum operasi adalah 180.81 ± 97.37 μm dan setelah operasi 252.15 ±
31.80 μm (p= 0.000). Pada area perifer superior, ketebalan makula sebelum
operasi adalah 227.19 ± 69.81 μm dan setelah operasi 269.48 ± 25.60 μm
(p=0.005). Ada perubahan signifikan dalam volume makula sebelum operasi
adalah 7.85 ± 1.78 μm3 dan setelah operasi adalah 9.46 ± 1.05 μm3 (p
=0.000). Tidak ada korelasi antara APT / EPT dan struktur seluler makula
(p>0.01). Tajam penglihatan lebih baik (p=0.000) dan penurunan TIO (p=
0.000) juga diperoleh secara signifikan dalam penelitian ini.
Kesimpulan: Perubahan ketebalan makula di area parasentral temporal,
perifer temporal, perifer superior dan volume makula dapat digunakan
sebagai biomarker baru untuk mengukur efek dari fluktuasi TIO dan manuver
pada operasi intraokular. Pada penelitian ini tidak didapatkan korelasi antara
APT / EPT dan perubahan struktur makula dalam fakoemulsifikasi.
Fakoemulsifikasi membantu meningkatkan tajam penglihatan, menurunkan
TIO dan mungkin aman untuk struktur seluler retina. Pemeriksaan SD OCT
disarankan dilakukan sebelum dan sesudah fakoemulsifikasi untuk menjamin
penglihatan seumur hidup.
