Kualanamu International Airport, located in Deli Serdang Regency, North Sumatra Province, is an airport with a hub type characteristic, namely as a connecting center to continue flights to other areas abroad. Operational activities at the airport will increase the volume of domestic liquid waste generated to cause new potential problems environmental pollution in the domestic liquid waste of Kualanamu International Airport. Domestic wastewater parameters routinely tested by Kualanamu International Airport management are COD (Chemical Oxygen Demand) and BOD (Biological Oxygen Demand) content contained in domestic wastewater. Some parameters have never been tested is the heavy metals characteristics in domestic liquid waste Kualanamu International Airport. In this study, domestic wastewater treatment at Kualanamu International Airport uses water lettuce (pistia stratiotes) and uses NPK fertilizer with variations of 1 g, 2 g and 3 g with an aeration rate of 3 L/min and difference in contact time variations for 24 hours and 48 hours. The results showed that the water lettuce with the addition of 3 g of NPK fertilizer, aeration rate of 3 L/min, and contact time 48 hours was the most efficient in treating domestic wastewater at Kualanamu International Airport. The maximum efficiency of reducing BOD is 64.965%, COD is 64.967%, Fe is 86.53 %, Oil and Fat is 75 %. The optimal efficiency of Zn is 85.75 % with the addition NPK fertilizer of 2 g, aeration rate of 3 L/min and contact time of 48 hours and the efficiency optimum reduction of TSS with contact time for 24 hours.
