Abstract

Managing an environment is a very serious and crucial matter. Accordingly, a simultaneous management and policy is needed to be done and applied. The regulation no.23/1997 about the management of environment (PLH) was decided as a basic law 10 evaluate and adjust all products which contain the regulation about environment, including the management of hospital environment. It is in accordance with the concept of Health Development Model, which concludes that the life and safety of human and other living creatures depends on healthy environment. Some factors such as behavior and way of life, genetic, and the health services are include, Hospital is one of the health service institutions which considers as one of the disease disseminator agents in society due to the un-optimal waste management of stakeholder. The government policy in conducting the environment management in hospital is by determining The Hospital Health Requirement by the Minister of Heakh Affairs in a decree 1n0.1204/Menkes/SK/X/2004 and Hospital Sanitation Guidelines in Indonesia by the Department of Health Affairs Republic Indonesia.

Arifin Achmad Regional General Hospital in Pekan Baru, Riau Pprovince, has {gone through a tremendous development since the reformation time. Since becoming B level Hospital Education, besides being recommended for its health service, The Arifin Achmad General Hospital is also recommended in the field of education and technology in health. The waste management which is one of the requirements of health environment hospital requires a comprehensive system in achieving a good result that can be useful to al, both hospital and even society. It is, not only creating healthy environment, but also can create continuous development with environmental concept

The studies conducted to acknowledge the waste management system in Arifin Achmad General Hospital, is an observational study using primary data resulted ffom an interview, questioner, and observation. Whereas, the secondary data was gained from a report of Arifin Achmad medical records. The sampling technique was based on the research consideration and limited on certain point. Some of the division head and the cleaning service are the responder in the research on waste problem in Arifin Achmad General Hospital. The result of the interview, questioner, and observation shows that the waste management system in Arifin Achmad General Hospital is stil on the document agenda base. Respondents gave answer that they do not know about the hospital waste management system. The socialization about the AMDAL of Arifin Achmad hhospital document thoroughly describes the waste management's commitment of the hospital which must always be given to all the officers of health. Officer who

directly involve with the waste management problem. Seeing from the techniques and efforts that have been done in managing the wast, it is obvious that these still need serious arrangement and a lot of commitment from every one in Arifin Achmad Hospital The lack of special team in managing the waste problem that will be directly responsible to the hospital director could be a serious barrier in handling the waste problem. Beside that, because of the limited fund for the waste management especialy for the routine activity, the liquid and gas waste quality checked, the hospital is not doing an independent observation yet and periodically test in accordance with the regulation in the decree minister of Health Department Affairs of Republic Indonesia (Kepmenkes RI) No.1204/Menkes/SK/2004 about the requirement of the hospital with healthy environment. From the quality analysis of the waste, parameter PO, are still having problem, because itis above the standard quality which is big enough, 2,74 mg/l. Primer and secondary phase in managing the wast is not quite enough to reduce the phosphate. Management must be continued to the third phase by the sediment process. Sediment with chemical way which is usually use to sediment phosphate.
