Coronary atherosclerotic disease is the leading cause of mortality and
morbidity in both established and developing countries. Acute coronary syndrome
(ACS) in form of acute ST segment elevation myocardial infarction (STEMI) is one
of the manifestations that associated with high mortality rate in the first 30 days.
Nowadays, the usefulness of platelet-lymphocyte ratio (PLR) has been studied as
predictor of death in STEMI patients.
The aim of this study is to assess the usefulness of PLR as a long term
predictor against major cardiovascular events (MACE) after 6 months from first onset
of STEMI
This in a retrospective study that enrolled 100 patients that diagnosed with
STEMI. Secondary data were obtained from RSFIAM medical record and the value of
PLR was calculated using the first laboratory result when patients were admitted to
the CVCU ward. Patients were categorized as low PLR group (s144) and high PLR
group (>144). Follow-up were done in all patients for 6 months and death, heart
failure, arrhythmia and reinfarction were counted as MACE.
From data analysis, we found 53 patients (53%) having MACE and among
that we found 33 patients (62,3yo) is in high PLR group. We also found significant
correlations between high PLR and death (p: 0,010; OR: 3,778;95o/o CI:1,461-
9,767), high PLR and reinfarction(p: 0,031; OR: 1,081;95Yo CI:0,990-1,181), and
high PLR and total MACE (p <0,001; OR: 9,429;95%oCI:3,552-25,03).
We can conclude that PLR is a useful long term independent predictor
againstMACE, particularlydeath and reinfarction in STEMI patiens
