Background. Heart failure occurs when the heart cannot ensure blood and oxygen to support other organs in the body. Heart failure is a serious condition, but it doesn't mean the heart has stopped beating. One of the risk factors for heart failure is hypertension because hypertension increases the work of the heart which induces structural and functional changes in the myocardium, these changes include left ventricular hypertrophy, which can progress to heart failure. Aim. Knowing the relationship between hypertension and heart failure in hypertensive patients at Haji Adam Malik Hospital. Methods. This research is an analytic observational study with a cross-sectional study design. The sample in this study was selected with the consecutive sampling method and obtained as many as 95 people. Data collection from medical records and analyzed by SPSS application, both from univariate and bivariate (chi-square test). Results. In this study, 66 people (69,5%) suffered from hypertension and 70 people (73,7%) suffered from heart failure. The results of the analysis of the second variable with the chi-square test obtained a p value of 0,001 which means there is a relationship between hypertension and heart failure in hypertensive patients at Haji Adam Malik Hospital. Conclusion. Heart failure due to patients suffering from hypertension is associated with the incidence of hypertension.
