Background: Immunization is one of the health efforts to stop the transmission of PD3I (Immunization-Preventable Diseases). Childhood immunization is mandatory for children after birth until they reach a certain age. According to WHO, in 2021, there has been a decrease in global immunization coverage in recent years, from 86% in 2019 to 83% in 2020. Purpose: to analyze a comparison of compliance of parents from different backgrounds in Kota Medan and Kota Negeri Sembilan to include their child in the mandatory immunization program. Methods: this research is a descriptive study using a cross-sectional approach and using a sample of the population of parents in Kota Medan and Kota Negeri Sembilan with children under five years of age. The analysis was carried out using the chi square test from SPSS to see the relationship between the dependent and independent variables. Results: there is a significant relationship between the factors of parents' employment status (p=0.012) and the support from family (p=0.026) in Kota Medan. Conclusion: there is a significant relationship between the factors of parents' employment status and support from the family with the completeness of the mandatory immunization of children in the Kota Medan, while there is no significant relationship between the factors examined and the completeness of mandatory immunisation of children in Kota Negeri Sembilan.
