Background: Onychomycosis is an infection of the nail component caused by a fungus. Candida onychomycosis is a nail infection caused by Candida spp. The selection of antifungal types to treat onychomycosis requires several considerations, ranging from efficacy, side effects, drug interactions, and the presence of comorbid factors. Fluconazole is a commonly used antifungal whereas voriconazole is recommended for the treatment of candida onychomycosis in cases refractory to or resistant to other antifungals. Objective: To know the comparison of fluconazole sensitivity test with voriconazole in onychomycosis caused by Candida species. Subjects dan Methods: This study is an in vitro experimental study with consecutive sampling method involving 30 patients at the KSM dermatology and venereology polyclinic, H. Adam Malik Hospital, Medan. Subjects with clinical features of onychomycosis, took anamnesis, physical examination, dermatological examination, and nail specimen sampling and sent to the clinical microbiology laboratory of H. Adam Malik Hospital Medan. Then the examination was carried out with a 20% KOH solution and then continued with the examination of fungal culture and fluconazole and voriconazole sensitivity test. The collected data were analyzed using McNemar and Kruskal Wallis tests. This research has received approval from the Research Ethics Commission of the University of North Sumatra and H. Adam Malik Hospital Medan. Results: Candida onychomycosis was more common in women (80%) with the age group of 46–55 years (36.7%) and cleaners and assistants (36.7%). The results of the negative KOH examination (76.7%) were more than the positive ones. The culture results showed that the most Candida species was C. albicans (53.3%). The results of antifungal sensitivity test using fluconazole (96.7%) showed sensitive results. The results of the antifungal sensitivity test using voriconazole were (100%) showing sensitive results. There was no significant difference in sensitivity between fluconazole and voriconazole (p=1,000), whereas the difference in sensitivity for fluconazole based on culture results showed a significant difference (p=0.039), voriconazole sensitivity based on culture results showed sensitivity for all Candida species. Conclusion: There was no significant difference in sensitivity between fluconazole and itraconazole against onychomycosis caused by candida species.en_US
