Irrigation is an important factor affecting root canal treatment. The use of irrigation materials currently does not meet the ideal requirements. Kersen leaf extract contains chemical compounds such as alkaloids, flavonoids, saponins, tannins, steroids, and terpenoids, which are expected to be alternative irrigation materials. This research is a laboratory experimental study with a post-test only control group design. A total of 24 teeth were grouped into 4 treatment groups. Preparation technique used was crown-down pressureless using a ProTaper Ni-Ti Rotary Instrument and irrigated using a 30G one-sided vented irrigation needle. Group I was irrigated with 15% cherry leaf ethanol extract, group II was irrigated with 15% cherry leaf ethanol extract + 2.5% NaOCl, group III was irrigated with 15% cherry leaf ethanol extract + 17% EDTA, and group IV was irrigated with NaOCl 2.5% + 17% EDTA. After preparation and irrigation, the sample was split in the buccolingual direction. Then the Scanning Electron Microscope (SEM) test was performed on the the apical third. Determination of the smear layer cleanliness score was determined by Torabinejad's score (2003). Data analysis used is Kruskall-Wallis and Mann Whitney. The results of the Kruskall Wallis test showed significant differences between groups with p<0.05 (p=0.000). The results of the Mann Whitney test showed no significant difference P>0.05 (p=0.475) between group 3 and group 4. However, groups 1 and 2 had a significant difference from group 4 where P<0.05 (p=0.000). The order of the groups that cleaned the smear layer the best were group IV, group III, group II, and group I.en_US
