The hook industry produces good quality hook products through improvements in the production process. The problems faced by PT. PSPI, is that there are variations in the quality standard for fishing rod strength 1053 size 10, which results in loss of quality. Based on this problem, it is necessary to calculate losses due to variations in the strength of the hooks in order to minimize company losses. This study uses the Taguchi’s Loss Function and Fault Tree Analysis (FTA) methods. Based on the calculations, it was found that the process capability index obtained Cp = 0.4125 where the process is not yet capable of specified standards. Calculation of the loss of quality of 1053 size 10 hooks products is IDR Rp2.228,87/unit per month. Based on the Fault Tree Analysis of the two top events, the root cause of the loss of quality is obtained, namely tampering temperature is not appropriate, and low carbon content in raw materials. Proposals for improving the production process given are provide evaluation and audit of operator performance and ensure operators understand the use of tampering machines, ensuring that the wire sent by the supplier is in good condition and there are no jagged on each circle of wire, and ensuring that the wire sent by the supplier is in good condition both with a carbon content of >70%, and checking storage warehouses periodically.
