Introduction : Singh index is classification of trabecular pattern on proximal femur which be observed with x-ray. This method can be used to detect an osteoporosis. Smoking has been found since 1976 that being able to decrease bone density. Many research have been done to see the affect of smoking to bone density which measure by DEXA-scan, but there were not much research of it by using Singh index. Material & Method: Ihis is a cross sectional study petformed at Haji Adam Malik Medan Hospital. Samples were divided into two groups, smoking and non-smoking groups with 43 persons in each group. Every person in the smoking group were noted the cigarette type, duration of smoking, & the frequency of smoking. Then the Singh index value was evaluated from the pelvic x-ray in all samples. Results : There was a significant correlation between smoking & the Singh index value (p=0,002). The duration of smoking directly affect the Singh index (p=0,001). But there were no correlation between the cigarette type (p=0,932) & the frequency of smoking (p=0,259) with the Singh index. Conclusion : It has been found that smoking has a significant correlation with the Singh index value, where smoking group has a lower Singh index value compare to non-smoking groupen_US
