Impaction and agenesis are the most frequently encountered dental anomalies in third molar. Impaction and agenesis third molar are associated dietary habits, evolutionary changes, and human jaw growth. The aim of the study was to radiographically evaluate the difference of impaction and agenesis third molar between vegetarian and nonvegetarian diets among the Medan population. The study included 30 selected panoramic radiograph (15 vegetarian radiographs and 15 nonvegetarian radiographs) used purposive sampling with an age range of 20–40 years. The obtained data were statistically analyzed using Chi-square test. The total impacted third molars were 63,3%. There were 66% impacted third molars in vegetarian and 61,7,7% impacted third molars in nonvegetarian. Mandibular third molar dominated in impaction. The distoangular was the most common angular pattern of impaction seen in maxilla and mesioangular in mandible of vegetarian and nonvegetarian. Class B was the most common pattern of impaction in maxilla and Level A in mandible. Class I was the most common type of impaction depth and ramus relationship. The overall third molar agenesis was 15%. There are 26,7% third molars in vegetarian and 3.3% in nonvegetarian were evaluated agenesis. Maxilla third molar dominated in agenesis. In the present study, variations of impaction and agenesis were found. The correlation between the dietary pattern and impacted third molar was found to be insignificant (p>0,05), but it was found to be significant for the the correlation between the dietary pattern and agenesis third molar (p<0,05)en_US
