Background: Acne vulgaris (AV) is a common inflammation of the pilosebaceous unit. Inflammation is one of the four important factors in the pathogenesis of AV. Brain-derived neurotrophic factor (BDNF) is a member of the neurotrophin family, a neurotrophic growth factor and plays a role in the inflammatory process in pathogenesis of AV. Durian peel extract (Durio zibethinus Murr) has been known to have antibacterial and anti-inflammatory activity against Cutibacterium acnes. It is suspected that applying durian peel extract can increase BDNF serum levels in AV. Objective: To determine the effect of durian peel extract on BDNF serum levels in AV. Subjects and Methods: This study is a clinical trial with pre-experimental with pretest – posttest design involving 31 subject ≥ 17 years old with AV. Recording of basic data and subject examination and measuring BDNF serum levels before and after applying durian peel extract. Data analyzed by paired T-test and Friedman test. Results: In this study AV was more dominant in women, 21 subjects (67.7%). Found that the highest age in the study group was aged 17 – 21 years, about 18 subjects (58,1%). The mean serum BDNF level before administration of durian peel extract was 32684.32 pg/ml, while after administration was 39088.94 pg/ml, showed a significant relationship (p<0.001). There was a decrease in the number of inflammatory and noninflammatory lesions in acne vulgaris after being given durian peel extract, each showed a significant difference (p<0.001). Administration of durian peel extract did not cause any side effects at all in all research subjects (100%). All subjects (100%) said they were satisfied with the changes experienced. Conclusion: The use of durian peel extract (Durio zibethinus Murr) can affect the increase of BDNF serum levels in AV.en_US
