Background: Headache is a very common and bothersome condition that, despite widespread attention in both the scientific literature and popular media, is still poorly understood. Tension-type headache (TTH) is the most common headache with a prevalence of 30-78% worldwide. Fatigue and stress, resulting in less resolved and weak experienced by residents within 6 (six) months after the residency education begins. Fatigue and stress levels are known to be associated with the incidence of TTH.
Objective: To determine the correlation between fatigue and stress on the incidence of TTH in first semester resident doctors at the Faculty of Medicine, University of North Sumatra.
Research methods: This research is an observational study with an approach cross-sectional uses primary data sources from all FK-USU resident doctors for the first semester of August 2022 who work at the H. Adam Malik General Hospital in Medan, the Network Hospital, and the Faculty of Medicine, University of North Sumatra. Fatigue status is measured by a score fatigue severity scale (FSS), stress status was measured by Perceived Stress Scale (PSS – 10), TTH frequency was adjusted according to ICHD-3 criteria, and pain intensity was measured accordingly Numeric Ratting Scale (NRS). To assess the relationship between fatigue and stress on the occurrence of tension-type headache, Spearman's correlation test was used.
Results: There were 98 research subjects who met the inclusion and exclusion criteria where the majority of subjects with TTH were male (59.7%), were in the age group <30 years (58.2%) with an average age of 29.7463 ± 3.59858 ( years), Batak ethnicity (52.2%), married (61.2%), and have children (56.7%). The majority of subjects with TTH were non-surgical study programs (68.7), while in the surgical study program the majority of TTH cases were General Surgery residents (11.9%); with a guard schedule <10x/month (64.2%). The majority of subjects experienced frequent TTH (41.8%), mild fatigue status (56.1%), moderate stress levels (76.5%), and moderate pain intensity (43,9%). The results of the statistical analysis found a significant relationship between fatigue and stress status on the frequency of TTH with a weak correlation strength (p=0,003, r= 0,294); there is a significant relationship between stress on the frequency of TTH with a weak correlation (p=0.023, r= 0.230,). There is no significant relationship between fatigue on pain intensity(p=0,160, r= 0,143) and There is no significant relationship between stress on pain intensity (p= 0,055, r=0,194).
Conclusion: There is a significant relationship between fatigue and stress status on the frequency of TTH with a weak correlation strength. However, there was no significant relationship between fatigue and stress on pain intensity of TTH.
