Background: The assessment of ventricular enlargement (hydrocephalus) in most cases is done subjectively and based on radiologist’s experience. Objective assessment of ventricular size can be done by assessing Evan's Index (EI) on CT scan. EI is the ratio of the transverse diameter of anterior horn of lateral ventricle to the largest internal diameter of cranium at the level of foramen Monro in axial projection. The normal value of EI based on previous studies is < 0.3. Age and gender affect the ventricular size. Objective: To analyze the average value of Evan's Index and its correlation with age and gender in patients who performed head CT scan at RSUP H. Adam Malik Medan. Method: Case series study on adult patients who performed head CT scan with normal findings at RSUP H. Adam Malik Medan from November to December 2022. Data collection of head CT scan results from databases in the period 2021-2022 as well as age and gender data from medical records was collected. Results: From 110 adult individuals who studied (49% male, 51% female), mean EI was 0.26 + 0.03 and mean age was 41.86 + 17.49. There was a significant correlation between age with EI (r: 0.442) and p<0.001 along with the effect of increasing age groups on increasing EI values. There was no significant correlation between gender and EI (p: 0.121; ηp2: 0.022). Conclusion: The average value of Evan's Index (EI) in adult individuals who were examined by head CT scan at RSUP H. Adam Malik Medan was 0.26 ± 0.03. There was a linear correlation between increasing age groups and increasing EI values. There was no difference in EI values between male and female individuals in this study.
