    Abstract
Introduction: Diabetic nephropathy is a serious complication of diabetes mellitus (DM) such as cardiovascular disease, and progression to End Stage Renal Disease (ESRD) is associated with an increased risk of death. The neutrophil-lymphocyte ratio is also used as a marker of inflammation in diabetic nephropathy. The neutrophil-lymphocyte ratio of the relative neutrophil count divided by the relative lymphocyte count in diabetic nephropathy patients in % units was obtained from routine hematology results using EDTA blood samples and measured by flowcytometry with Sysmex XN-1000. The glomerular filtration rate is measured according to the Cockroft-Gault formula. Method: This research is an analytic observational study with cross sectional data collection method. This study used blood samples from internal medicine patients in the endocrine and nephrology department who were treated at H. Adam Malik General Hospital in Medan as many as 30 patients. Samples were checked only once for NLR with eGFR values. This research was conducted after getting ethical approval and informed consent. Results : In this study, of the 30 patients, there were 19 men (63.3%) and 11 women (36.7%). The mean age of the subjects was 58.04 years with the youngest being 46 years old and the oldest being 69 years old. The mean NLR score was 7.89 with the lowest score 2.11 and the highest 23.68. The average eGFR was 22.33 mg/dL with the lowest value being 14 mg/dL and the highest being 46 mg/dL. Spearman correlation test analysis of neutrophil lymphocyte ratio (NLR) values correlated weakly with no significant relationship between neutrophil lymphocyte ratio (NLR) and glomerular filtration rate (eGFR) in patients with diabetic nephropathy (r = -0.248, p = 0.186) . Conclusion: This study did not find a significant relationship between the neutrophil lymphocyte ratio (NLR) and the glomerular filtration rate (eGFR) in patients with diabetic nephropathy with a value of p=0.186.
