dc.description.abstractMedical record is a document file that contains records of patient identity, examination, action, treatment, and other services. For many years since the Pratama Kasih Bunda Clinic was founded, until now the process of data collection, services, and recording of patient medical records still uses written media. The system is not efficient because it can cause uncontrolled duplication of patient data so that errors often occur. The process of searching for patient data is very difficult and takes time to look for data one by one. The purpose and benefits of this research are intended to make it easier for medical staff or admins who work in the medical records section to be able to store and process data safely and in an organized manner. So as to increase the accuracy, accuracy of the data and facilitate management in managing reports related to medical records. This study designed a Website-based medical record storage application that would be designed using PHP and Mysql programming languages and using the waterfall or classic life cycle model as the design and development methodology in this research application.en_US
