Background: Autonomic dysfunction is one of the non-motor symptoms (NMS) in Parkinson's disease that can occur approximately 5-20 years earlier than the onset of motor symptoms. Autonomic dysfunction spanning the domains of urinary, gastrointestinal, sexuality, cardiovascular, pupilomotor and thermoregulation all of which have an impact on the quality of life of patients, an increasing number of clinical studies have appeared in recent years. Objective : To determine relationship of autonomic dysfunction, duration of diasease and treatment with severity of parkinson’s disease. Methods: This study used a cross-sectional design of Parkinson's patients who were treated at the Neurology Polyclinic of RSUP. Hajj Adam Malik Medan and Network Hospital in January -May 2023. Diagnosis according to United Kingdom Parkinson's Disease (PD) Society Bank's Clinical Criteria. Autonomic dysfunction was assessed using the Scales for Outcomes in PD for Autonomic Symptoms questionnaire, Parkinson's severity using the Unified Parkinson's Disease Rating Scale. Results: From the 37 study subjects the majority were male (56.8%), mean age 67.38 years, high school education, most jobs were housewives, ethnic Batak, the majority of subjects had no family history of Parkinson's disease. Hypertension is the most comorbid. Most are in stage I (32.4%). Having a disease duration of 2 – < 11 years, approximately 54.1% had taken medication for < 5 years and most received a Levodopa combination drug (97.3%). There is a significant relationship between autonomic dysfunction and the severity of Parkinson's disease (p <0.001) with (r= 0.814), There is a relationship between the duration of the disease and the length of treatment with the severity of Parkinson's (p <0.001) with( r = 0,820) dan ( r = 0,764). Conclusion: There is a positive correlation of autonomic dysfunction, disease duration and treatment with the severity of Parkinson's disease.en_US
