T. BIA is one of the compnies engaged in the manufacturing of plywood. During the production process of its products, several defects have been found due to various factors, one of which is human error by the workers involved in the production process. The research aims to analyze human error in the wood industry suplly chain using the SCOR (Supply Cain Operations Reference) model. Observations were conducted on the operators workking at PT. BIA using the SHERPA (Systematic Error Reduction and {rediction) and HEART (Human Error Assessment and Reduction) methods. The SHERPA mothode was used to access tasks according to the level of human error at PT. BIA. Based on the calculation of the assessed effect (AE) values, the highest total AE was obtained in the manufacturing process with a value of 96.21. The highest Human Error Probability (HEP) was found in the wood milling, veneer cutting, sizing, and quality control processes, with a Human Error Probability (HEP) value of 15.39. The results of the study can be used as a reference to develop improvement plans to reduce human error.en_US
