    Abstract
    Human error is one of the causes of product quality degradation. The consideration of human error as a cause of product failure in the manufacturing process refers to human error as a cause of quality problems in several manufacturing industries. This research was conducted at PT. Medan Tropical Canning & Frozen Industries which is a company canning fish and frozen food. To be able to improve the industry, companies must improve performance, one of which is through supply chain management in the company. The role of humans in supply chain activities is very crucial because humans act as producers, workers, and consumers. This study aims to identify human error in the supply chain of canned fish PT. Medan Tropical Canning & Frozen Industries by adopting SCOR models, namely, plan, source, make, deliver, and return. Data processing is carried out using the SHERPA (Systematic Human Error Reduction and Prediction) method. The results showed that activities that had high errors and critical levels were in the plan process with errors in the accumulation of packaging materials (cans) and in the make process with errors in disposing of fish stomach contents, and errors in the preparation of fish in the rack. After analysis was carried out on each series of canned fish supply chains PT. Medan Tropical Canning & Frozen Industries was given a proposal for improvement in each supply chain activity. Companies must prioritize improvement of activities that have high errors and critical levels. By doing this, it is expected that supply chain performance can increase so that every supply chain activity can run smoothly, and products are in accordance with consumer wishes.
