Background: Postoperative pain is still a major problem for postoperative patients. Uncontrolled postoperative acute pain is associated with patient dissatisfaction, postoperative complications, and is a risk factor for developing chronic pain. One strategy to provide effective postoperative analgesia and to reduce unwanted side effects is the use of a combination of local anesthetics with epidural morphine. Although epidural morphine has also been used in general for postoperative pain management, there is little information regarding the analgesic effect of the combination of ropivacaine and morphine and its comparison with morphine.
Objective: Obtain a comparison of 2 mg Morphine + 0.9% Nacl with 2 mg Morphine and 5 mg Ropivacaine via an epidural catheter on a pain scale with a numerical rating scale (NRS)
Methods: The research design is randomized clinical trial with a double blind method. After obtaining approval from the Ethics Committee of the Faculty of Medicine, University of North Sumatra Medan, 24 research samples were collected which after fulfilling the inclusion criteria were divided randomly into 2 groups. Group A received 2 mg of Morphine with 0.9% Nacl and group B received 2 mg of Morphine + 5 mg of Ropivacaine. The research data obtained were tested using SPSS.
Results: The NRS T1 value in the group given Morphine 2 mg + 0.9% NaCl had a median value of 0. The NRS T2 value had a median value of 1. The NRS T1 value in the group with Morphine 2 mg + Ropivacaine 0.2% has a median value of 0. NRS T2 value has a median value of 0. There is a higher incidence of PONV in the 2 mg Morphine group + NaCl 0.9% on T1 and T2 but not statistically significant, and the incidence of motor block in the Morphine 2 mg group with Ropivacaine was 0.2% more on T1 but not statistically significant.
Conclusion: Administration of epidural anesthesia 2 mg Morphine + 5 mg Ropivacaine is superior to administration of 2 mg Morphine with 0.9% Nacl in reducing postoperative NRS.
