Background: The rationale reason behind using the Neutrophil-to-Monocyte Ratio to identify COVID-19 patients with a higher risk of death is due to the role that monocytes and neutrophils play during the immune response. Neutrophil and Monocyte hyperactivation can differentiate into macrophages playing an important role in regulating the inflammatory response that occurs in COVID-19 patients. Methods: This study was conducted retrospectively on 93 medical records of COVID-19 patients at H. Adam Malik General Hospital Medan who met the study inclusion criteria. Bivariate analysis was carried out to determine whether there was a relationship between NMR and the Severity of COVID-19 using the Mann-Whitney test and it was interpreted as significant if p < 0.05. Result: There was a significant difference in NMR values between moderate and severe patients (p = 0.006), as well as between moderate and critical patients (p = 0.001). However, there was no significant difference in NMR values between patients with severe and critical degrees (p = 0.077). Conclusion: The Neutrophil-to-Monocyte Ratio is a simple laboratory marker that can be an independent risk factor in predicting the severity of COVID-19en_US
