dc.description.abstractDischarge planning plays a highly crucial role in patient care from the arrival time of the patient at the hospital until the time they go home. Discharge planning can be effective in reducing hospital readmissions. This study aims to determine the implementation of Discharge Planning for inpatients at Universitas Sumatera Utara Hospital. This is a descriptive research design with a quantitative approach. The implementing nurses in the inpatient room of Universitas Sumatera Utara Hospital serve as the samples which are determined by using the Total Sampling technique. A total of 70 nurses are taken as samples. The research questionnaire consists of demographic data and a Discharge Planning questionnaire. The results show that the implementation of Discharge Planning is categorized as poor with a total of 47 nurses (67.1%) because the nurses sometimes have no time to provide health education due to their high workload. Nurses should carry out Discharge Planning for all patients with the purpose to prepare them for discharge where the patients are able to manage ongoing care at home.en_US
