Perbandingan Pre-Loading Cairan 20 cc/kgBB dan 30 cc/kgBB setelah One Hour Bundle terhadap Nilai Laktat pada Pasien Sepsis di Ruang ICU RSUP Haji Adam Malik
Abstract
Background: Per definition, sepsis is a life-threatening condition caused by abnormality or dysfunctionality of host-response to an infection, mostly with bacteria etiology. One of the most used biomarkers for organ dysfunction is serum lactate level, in which the higher the value is, may translate into higher risk for such dysfunction. The role of fluid therapy in sepsis condition during one hour bundle phase is unquestionably essential, ruling to resuscitate an individual within 20-30 cc/kgbw though the most optimal dose is yet to be determined.
Objective: To analyze the influence in pre-loading dose of 20 cc/kgbw and 3 cc/kgbw after one hour bundle phase to the lactate level among individual with sepsis in Haji Adam Malik General Hospital.
Method: This study use randomized clinical trial model with double blinding method. After receiving approval from ethical committee of Universitas Sumatera Utara, the author included 36 individuals with sepsis, in which further classified into 2 different groups i.e., group 1 (20 cc/kgbw) vs. group 2 (30 cc/kgbw). The results were processed by using SPSS software.
Results: There is not any difference in lactate level 6 hours after fluid administration (p=0,002). Nevertheless, there is a significant difference (P<0,05) in lactate clearance rate as observed in group 1 (32,55 ± 30,69 mmol/L) vs. group 2 (-15,62 ± 50,62 mmol/L).
Conclusion: The fluid administration dose of 20 cc/kgbw is more recommended to achieve higher and more optimalized lactate clearance rate in this study.
