Background: : Breast cancer is the leading cause of cancer death in women. From Globocan data for 2020, the number of new cases of breast cancer reached 68,858 out of a total of 396,914 new cases of cancer in Indonesia. Most breast cancer patients tend to be slow in receiving medical treatment at the hospital. This is because patients generally only come to health facilities when their breast cancer has reached an advanced stage. So the need for long treatment causes biopsycho-social-spiritual impacts. Breast cancer patients will experience physical and psychological changes because they have to adapt to new conditions in their lives. Adaptation to breast cancer is influenced by external factors (eg, social support and living conditions) and internal factors (eg, personal resources, self-defense mechanisms, and coping strategies). Personal resources include spirituality, optimism, and assertive behavior. Personal adjustment affects the quality of life of breast cancer patients. Methods: Sampling using non-probability sampling, consecutive sampling type, was carried out on 103 subjects with advanced breast cancer who were treated at the Surgical Oncology Polyclinic at H. Adam Malik General Hospital, Medan from July to August 2023. Results: A description of the demographic data of advanced breast cancer patients at H. Adam Malik General Hospital in Medan is that the age variable is mean ± standard deviation 50.85 ± 8.73, the highest employment variable is not working, 79 subjects (76.7%), the highest educational variable is High School or equivalent, namely 56 subjects (54.4%), the largest ethnic variable was Batak, namely 42 subjects (40.8%), and the highest marital status variable was married, namely 93 subjects (90.3%). The highest category of spirituality is high, namely 60 subjects (58.3%), the most anxiety is the severe category, namely 50 subjects (48.5%), the highest social support is the high category, namely 51 subjects (49.5%) and the quality variable Most of the lives were good, namely 50 subjects (48.5%). Using the Spearman test, we found a positive correlation between spirituality (SWBS) and quality of life (WHOQOL Bref), r = 0.719, n = 103 and p value <0.0001, a negative correlation between anxiety level (HADS) and quality of life (WHOQOL Bref). , r = - 0.915, n=103 and p value <0.0001, and a positive correlation between perceived social support (MSPSS) and quality of life (WHOQOL Bref), r=0.779, n=103 and p value <0.0001 Conclusion: There is a strong positive relationship and correlation between the level of spirituality and perceived social support with the quality of life of advanced breast cancer patients. There is a strong negative relationship and correlation between anxiety levels and the quality of life of advanced breast cancer patients.en_US
